Summary
Clinical course and laboratory tests were analyzed in patients with parathyroidectomy after successful renal transplantation and compared with respect to type of immunosuppression [azathioprine (n=6) versus cyclosporin A (n=9)j. The cyclosporin group showed marked hypercalcemia, hypophosphatemia, increase of alkaline phosphatase and parathormone, and elevated cyclosporin blood levels. We suspect that there is an interaction between cyclosporin A and parathormone that inhibits the regression of secondary hyperparathyroidism, thus leading to highly active renal osteopathy.
Zusammenfassung
Bei Patienten mit Parathyreoidektomie nach erfolgreicher Nierentransplantation wurden klinischer Verlauf and Laborwerte analysiert and hinsichtlich der Immunsuppression miteinander verglichen (Azathioprinn=6/Cyclosporin An=9). Die Cyclosporin-Gruppe zeigte ausgepräg-te Hypercalcaemie, Hypophosphataemie, Anstieg von alkalischer Phosphatase and Parathormon and erhöhte Cyclosporin-Blutspiegel. Eine Interaktion zwischen Cyclosporin A and Parathormon wird vermutet, die die Rückbildung des sekundären Hyperparathyreoidismus hemmt and so eine hochaktive renale Osteopathie unterhält.
