Perforated aneurysm of an aberrant right hepatic artery: bleeding from the papilla of Vater. Case report and review of the literature

Abstract

A rare case of spurious aneurysm of an aberrant right hepatic artery is reported. Endoscopic retrograde cholangio-pancreaticography, computed tomography and selective angiography of the coeliac trunc are diagnostic. Resection of the aneurysm and interposition of saphenous vein is the preferred procedure. In selected cases ligation of the common hepatic artery is possible. Intrahepatic aneurysms are preferably treated interventionally.

Zusammenfassung

Es wird über den ungewöhnlichen Fall eines entzündlich bedingten Aneurysma spurium einer aberrierenden rechten Leberarterie anhand von exemplarischem Bildmaterial berichtet. ERCP, Computertomographie und Zöliakographie ermöglichten die Diagnose. Neben gefäßchirurgischen Rekonstruktionen mit Resektion des Aneurysmas und Interposition der V. saphena magna kommt in ausgewählten Fällen auch die Ligatur der A. hepatica communis in Frage. Für intrahepatisch gelegene Aneurysmen kommen vorwiegend interventionelle Methoden in Betracht.
