Problematik der sogenannten angeborenen Leukämien (Leukämien, leukämoide Reaktionen, Reticulosen)

Summary

The problems of the so-called congenital leukaemia are demonstrated by the history of a newborn having the following symptoms: the whole skin was of a brownish-yellow colour with disseminated haemorrhagias and covered with small papular efflorescences from petechial up to nodular size. The main histological finding was a proliferation of reticular, monocytoid cells accompanied by focal cariorhexis. The same proliferation was found in the bone-marrow, where syncytial accumulation of these cells-also here with numerous mitosises and many striking nucleoli-dominated, cells of normal erythro- and granulocytopoiesis were almost completely absent. During the first weeks there was a leucocytosis in the peripheral blood with considerable percentage of monocytoid cells which may be classified most likely as reticular or histiocytic monocytes. The infant was in a good general condition. Hepato-splenomegaly, enlarged lymph nodes ore elevated temperature were not present. All the alterations described above disappeared after a few weeks without any therapy. As the child survived this disease primarly impressing as a neoplastic one must be considered to be a reactive congenital reticulosis of unknow aetiology. The problems of this case are discussed and a review of literature on congenital leukaemia, leukaemoid reactions and reticulosis is given.

Die Probleme der sog. angeborenen Leukämie werden an einem Neugeborenen aufgezeigt, das mit folgenden Veränderungen geboren wurde: Die gesamte Haut war braun verfärbt, mit fleckigen Hämorrhagien durchsetzt und übersät mit kleinen, petechienartigen bis zu knoten-förmigen, flach über die Haut vorspringenden Prozessen. Das Grundsubstrat war eine Proliferation reticulärer, monocytenartiger Elemente mit eingestreuter Karyorhexis. Die gleiche Proliferation fand sich im Knochenmark, in dem Verbände dieser Zellen-ebenfalls mit häufigen Mitosen und vielen auffälligen Nucleolen-das Bild beherrschten; Zellen der normalen Erythro- und Granulopoese waren kaum vorhanden. In den ersten Lebenswochen bestand außerdem im peripheren Blut eine Leukocytose mit erheblichem Anteil monocytoider Zellen, die am ehesten als reticuläre bzw. histiocytäre Monocyten einzuordnen waren. Der Allgemeinzustand war gut, Hepatosplenomegalie, vergrößert tastbare Lymphknoten und Fieber fehlten. Die beschriebenen Veränderungen bildeten sich ohne Therapie in wenigen Wochen zurück, das Kind überlebte. Dieser zunächst als blastomatös erscheinende Prozeß wurde als reaktive, angeborene Reticulose unbekannter Ätiologie angesprochen. Die Problematik dieses Falles wird innerhalb der Literatur über angeborene Leukämien, leukämoide Reaktionen und Reticulosen diskutiert.
