Zusammenfassung
Die klinische Erfahrung mit 11 Kindern (6 Mädchen, 5 Jungen) mit einer CHARGE-Assoziation wird dargestellt. Alle 11 Kinder wiesen einen komplexen Herzfehler auf. Eine beidseitige Choanalatresie bestand bei 8 Kindern. Kolobome an einem oder beiden Augen sowie Ohrmuscheldysplasien bzw. Hörminderungen wurden in 9 Fällen beobachtet. Neben diesen Leitsymptomen konnten im orofazialen Bereich weitere Fehlbildungen und Funktionsstörungen festgestellt werden. So litten alle Kinder an schweren velopharyngealen Koordinationsstörungen mit rezidivierenden Aspirationspneumonien. Bei 3 Patienten bestand eine LKG-Spalte bzw. eine isolierte Gaumenspalte. Eine Mikrognathie bzw. einen gotischen Gaumen fanden wir in je 2 Fällen. 6-mal war eine Fazialisparese zu beobachten. Die Patienten mit CHARGE-Assoziation erfordern ein interdisziplinär koordiniertes Behandlungskonzept. Die Beseitigung der Choanalatresie sollte so früh wie möglich erfolgen. Wir bevorzugten in 6 von 8 Fällen den transpalatinalen Zugang. Dieses Vorgehen ermöglicht eine gute Übersicht und anatomisch korrekte Rekonstruktion. Mit der Stabilisierung der Kinder wird eine begleitende Langzeitbehandlung notwendig, die der Entwicklungsförderung und Überwindung der Kau- und Schluckstörung dient.
Summary
Clinical experience of 11 patients with CHARGE association is reviewed. The study comprised six girls and five boys. All presented with congenital heart disease. Eight had bilateral choanal atresia. Coloboma affecting either one or both eyes was detected in nine children. Nine patients suffered abnormal pinnae or deafness. In addition to major abnormalities, further anomalies of the orofacial region and the upper airways occurred in all patients. All children exhibited velopharyngeal incoordination and swallowing problems often resulting in recurrent aspiration pneumonia. Three patients had cleft lip and palate. Both micrognathia and high-arched palate were present in two patients. Facial palsy was observed in six patients. Optimal management requires a multidisciplinary approach with active coordination and cooperation between the appropriate specialities. The anatomical repair of bilateral choanal atresia should be performed as early as possible. In our experience, the transpalatal approach is far more satisfactory than the transnasal. It gives the surgeon the opportunity of direct access and anatomical repair. After the child has grown stronger, a long-term management is desirable including developmental and feeding training.
