Zusammenfassung
Es werden 40 Patienten mit röntgenologisch nachgewiesener prämaturer Kraniosynostose kephalometrisch untersucht. Die am seitlichen Fernröntgenbild gemessenen Längen- und Winkelwerte werden mit altersentsprechenden Normwerten verglichen. Auffallende Verkürzung finden sich bei der Strecke Na−S in über zwei Drittel der Fälle, geringgradige Verkürzungen der Strecken Na−Ar, S−Ba, ANS-PNS, Pogo-Go bei etwa der Hälfte der Patienten. Hingegen entspricht die Länge der Strecke S−Bo meist der Norm. Als Erklärung der abnormen kephalometrischen Meßwerte werden zwei Ursachen angenommen: 1. Wachstumsstörungen im Bereich der Suturen und Synchondrosen der Schädelbasis und des Gesichts, 2. prämature Schädeldachnahtsynostosen und eine intrakranielle Drucksteigerung.
Summary
Forty patients with radiologically verified premature craniosynostosis underwent cephalometric examination. Distances and angles obtained on telecephalometry in the lateral projection were compared with age averages. In more than two thirds of the cases the Na−S distance was found to be markedly shortened, while about half of the cases showed minor shortening of Na−Ar, S−Ba, ANS-PNS and Pogo-Go. By contrast, S−Bo was usually within normal. These abnormal cephalometric data were thought to be attributable to two causes: 1. Defective growth involving sutures and synchondroses of the base of the skull and the face. 2. Premature calvarian synostosis and increased intracranial pressure.
Résumé
On a examiné céphalométriquement 40 malades présentant une craniosténose prématurée confirmée radiologiquement. Les longuers et angles mesurés sur la téléradiographie sagittale sont comparés avec des valeurs normales de sujets d'un âge correspondant. Dans plus de deux tiers des cas on trouve des raccourcissements nets du segment Na−S et chez la moitié des malades des raccourcissements mineurs des segments Na−Ar, S−Ba, ANS-PNS, Pogo-Go. Par contre, la longueur du segment S−Bo est généralement normale. Deux causes peuvent expliquer les valeurs céphalométriques anormales: 1) les troubles de la croissance au niveau des sutures et synchondroses de la base du crâne et de la face; 2) les synostoses prématurées de la suture de la voûte crânienne et une augmentation de la pression intracrânienne.
