Chronische primäre alveoläre Hypoventilation

Zusammenfassung

Es wird über eine Patientin mit chronischer alveolärer Hypoventilation infolge herabgesetzter Empfindlichkeit des Atemzentrums auf CO2 berichtet. Nach Ausschluß der bekannten Ursachen muß eine primär zentrale Genese angenommen werden, wobei möglicherweise eine rezidivierende Blutung aus einem röntgenologisch nachgewiesenen Carotisaneurysma eine Rolle spielt. Obwohl die Patientin nach Atmung sauerstoffangereicherter Luft eine Verschlechterung der Blutgaswerte erkennen läßt, scheint den Chemoreceptoren des Carotissinus und des Aortenbogens in diesem Falle nicht die alleinige Bedeutung für die Atemregulation zuzukommen. Infolge des stark wechselnden arteriellen Sauerstoff- und Kohlensäurespiegels kommt es zeitweise zu Krampfstromentladungen im Elektrencephalogramm, die als stoffwechselbedingt aufgefaßt werden.
