Zusammenfassung
Zur quantitativen Erfassung von Blutverlusten aus dem Gastrointestinaltrakt markierten wir die Patienten-Erythrocyten mit Chrom 51 und bestimmten die Chrom 51-Aktivität im 24 StdStuhl. Die Messungen erfolgten mittels Szintillationszähler in einem Bohrlochkristall (60 × 60 mm Natriumjodidkristall) in einer Ringschaleneinheit in Glasbehältern mit einem Fassungsvermögen bis 0,5 Liter. Die Menge des Blutverlustes wird aus dem Verhältnis der Stuhlaktivität zur Blutaktivität errechnet. Die trotz gleicher Aktivität verschiedene Meßausbeute je nach der Stuhlmenge glichen wir durch Faktoren, die die Füllhöhe in den Behältern berücksichtigen, so weit aus, daß wir auf das sehr aufwendige Homogenisieren des Stuhles meist verzichten konnten. Der normale tägliche Blutverlust aus dem Gastrointestinaltrakt beträgt im Mittelwert 0,78 ml, die 2 σ-Grenze beträgt 0,78±1,1 ml. Blutverluste aus dem Gastrointestinaltrakt bei verschiedenen Erkrankungen konnten quantitativ bestimmt werden. Die Bedeutung der Methode für die klinische Diagnostik wird besprochen.
Summary
In order to obtain a quantitative estimation of blood loss from the gastro-intestinal tract the patient's erythrocytes were tagged with chromium 51 and the activity of this substance was estimated in the 24-hour faeces. Measurements were made with a sctintillation counter in a bore-hole crystal (60 × 60 mm sodium iodide crystal) in a circular unit scale in glass containers with a content of 0.5 litre. The amount of blood loss is calculated from the ration of faecal activity to blood activity. The different readings due to the quantity of faeces that were found in spite of equal activity were equalized by factors that took into account the height of the contents of the container, so that great homogenization of the faeces could be dispensed with. The mean value of normal daily loss of blood from the gastro-intestinal tract is 0,78 ml, 2 σ=0,78±1,1 ml. It was found possible to estimate gastrointestinal blood loss in a variety of diseases. The value of the procedure in diagnosis is discussed.
