SZusammenfassung
Es wurde geprüft, inwieweit durch Bestimmung von Serum-Kreatin-Phosphokinase und Serum-Aldolase Heterozygote der recessiv x-chromosomalen Muskeldystrophie mit bösartigem und gutartigem Verlauf (Typ IIIa und IIIb) identifiziert werden können. Die Untersuchungen wurden an 17 Familien mit insgesamt 22 Patienten vom bösartigen Typ und an einer ausgedehnten Sippe mit gutartigem Typ durchgeführt. Beim Typ IIIa hatten sechs von sieben sicheren Genträgerinnen eine erhöhte Kreatin-Phosphokinase. Von zehn möglichen Genträgerinnen erwiesen sich zwei aufgrund der erhöhten Kreatin-Phosphokinase als sichere Konduktorinnen. Von den zwölf sporadischen Fällen des Typ IIIa sind nach den Serumenzymbefunden bei den Müttern zehn als echte sporadische Fälle, also Neumutation bei der Mutter, aufzufassen. Beim Typ IIIb hatten sieben von neun sicheren Konduktorinnen und zwei von sechs möglichen Genträgerinnen eine erhöhte Kreatin-Phosphokinase. Die Kreatin-Phosphokinase kann nach den heutigen Erkenntnissen als sicherste Methode zur Identifizierung von Heterozygoten der recessiv x-chromosomalen Muskeldystrophie angesehen werden. Der Aldolase kommt nur ein begrenzter Wert zu.
Summary
32 certain or possible female carriers of the gene for severe recessive x-chromosomal muscular dystrophy (type IIIa or type Duchenne) and benign recessive x-chromosomal muscular dystrophy (type IIIb or type Becker) were examined clinically and biochemically. The creatine-phosphokinase was elevated in 6 of 7 certain carriers (type IIIa) and 7 of 9 certain carriers (type IIIb). Out of 16 possible carriers of type Duchenne 2 were identified as certain carriers by elevation of creatine-phosphokinase. Similarely 2 of 6 possible carriers of type Becker had elevated creatine-phosphokinase. Among 12 sporadic cases ofDuchenne type 10 were thought to be true sporadic cases due to de novo mutation in the mother, because the serum creatine-phosphokinase was normal. Determination of creatine-phosphokinase activity is probably the best method for idendification of heterozygotes of recessive x-chromosomal muscular dystrophy.
