Zusammenfassung
Bei insgesamt 18 Patienten mit verschiedenen Graden von Niereninsuffizienz wurden Blut- und Urinspiegel des biologisch aktiven Chloramphenicols sowie der Chloramphenicol-Abbauprodukte bestimmt. Das biologisch aktive Chloramphenicol wurde mittels einer mikrobiologischen Methode bestimmt, als Testkeim diente ein Stamm von C. diphtheriae. Die Abbauprodukte des Chloramphenicols wurden polarographisch ermittelt. Es konnte gezeigt werden, daß die Spiegelhöhe der Chloramphenicol-Abbauprodukte mit dem Grad der Niereninsuffizienz zunimmt. Unter einer 14 Tage lang fortgesetzten Behandlung mit Chloramphenicol kommt es jedoch nicht zu einer fortlaufenden Steigerung dieser Spiegel (auch nicht bei extremer Einschränkung der Nierenfunktion und einer Dosierung von 2–3 g), vielmehr wird eine Serumkonzentration erreicht, die nur noch geringe dosisabhängige Schwankungen aufweist. Die Befunde vonKunin u. Mitarb., daß die Serumspiegel des bakteriologisch aktiven Chloramphenicols von dem Grad der Nierenfunktionseinschränkung unabhängig sind, konnten wir bestätigen. Jedoch nehmen die Urinkonzentrationen des BAC bei eingeschränkter Nierenfunktion erheblich ab. Die Clearance des biologisch aktiven Chloramphenicols beträgt bei den verschiedenen Formen der Niereninsuffizienz etwa 20% der Inulin-Clearance. Die Clearance der Abbauprodukte des Chloramphenicols wurde stets höher als die Inulin-Clearance gefunden, aber niedriger als die PAH-Clearance, was dafür spricht, daß diese Abbauprodukte auch tubulär ausgeschieden werden. Die Konsequenzen der Untersuchungsbefunde für die Chloramphenicol-Therapie bei Patienten mit Niereninsuffizienz werden erörtert.
Summary
Urine and blood levels of the biological active chloramphenicol and chloramphenicol matabolics were estimated in 18 patients with different degrees of renal failure. The biological active chloramphenicol was estimated by a microbiologic method. The test was made with a strain of C. diphtheriae.
The metabolics of chloramphenicol were found polarographical. There was to be seen that the level of the chloramphenicol metabolics increases with the degree of renal failure. But during a fortnight-treatment with chloramphenicol there is no continuous increase of these levels (even if renal function is extremely restricted and chloramphenicol is applicated in a dosis of 2–3 g). Moreover the serum-concentration of the metabolics are approaching a level, which shows only little variation depending on the dose. We could confirmKunin's et al. findings that the serum-levels of the biological active chloramphenicol are independant of the degree of restriction of renal function. The urine-concentrations of bacteriological active chloramphenicol diminish considerably if renal function is restricted. The clearance of the biological active chloramphenicol in the different forms of renal failure is about 20% of the Inulin-clearance. The clearance of the metabolics of chloramphenicol was always found higher than the Inulin-clearance but lower than the PAH-clearance. This means that these metabolies are also excreted by the tubulus.
The consequences of these findings for the chloramphenicol-therapy in patients with renal failure are discussed.
