Zusammenfassung
1. Bei 44 Patienten mit durch Rectumbiopsie oder Autopsie gesichertem Amyloidbefall wurden in der Zeit von 1964–1967 elektrophoretische und immunologische Serum- und Urineiweißuntersuchungen durchgeführt.
2. Diagnostiziert wurden 21 erworbene (sekundäre) Amyloidosen mit periretikulärer Amyloidablagerung, 8 erworbene Amyloidosen mit perikollagener Amyloidablagerung, 2 hereditäre und 8 idiopathische Amyloidosen mit perikollagener Amyloidablagerung und 5 Mischformen mit periretikulärer und perikollagener Amyloidablagerung.
3. In 9 von 44 Fällen (20,5%) ließ sich ein Paraprotein sichern. Unter diesen befanden sich 8 Patienten mit perikollagener und 1 Patient mit perikollagener und periretikulärer Amyloidose.
4. Von 35 Patienten mit verschiedenen Paraproteinämien konnten bei 9 (26%) histologisch Amyloidablagerungen nachgewiesen werden.
5. Ein für die Amyloidose typisches Immunglobulinspektrum liß sich bei unseren Fällen nicht beobachten.
6. Die Konstellationstypen des Serumeiweißbildes wurden — besonders auffällig bei den erworbenen Amyloidosen — durch das Grundleiden beeinflußt.
Summary
1. The serum and urinary proteins in 44 patients with deposits of amyloid have been studied by paperelectrophoresis, immunoelectrophoresis, starchgelelectrophoresis and radial immunodiffusion. Rectal biopsy was performed on all patients during the years 1964 through 1967. Amyloid was demonstrated in all cases by polarisationsmicroscopy.
2. According toMissmahls classification 21 patients had acquired (secondary) A. of the perireticulin type, 8 patients hat accquired A. of the pericollagen type, two had hereditary and 8 suffered from idiopatic A. of the pericollagen type and 5 A. of perireticulin and pericollagen type (mixed form).
3. In 9 of 44 cases (20,5%) paraproteins were present. 8 of these showed the pericollagen type and one perireticulin and pericollagen type of A.
4. Amyloidosis was determined in 9 (26%) of 35 cases with paraproteinaemia.
5. The great diversity of electrophoretic patterns in the different types of A. has been demonstrated. The electrophoresis is mainly determined by the underlying primary disease.
6. Atypical distribution of immunoglobulins in A. was not observed.
