Zusammenfassung
Bei 24 Patienten mit histologisch gesicherter chronischer Hepatitis wurde in einer programmierten Studie nach der Methode der zufälligen Gruppenzuordnung die Wirksamkeit einer immunsuppressiven Behandlung mit Leukeran gegenüber einer Kontrollgruppe mit nur Laevocholin- und Vitamin-Medikation untersucht. Die Beurteilung der Therapieresultate erfolgte bei allen Patienten nach dreimonatiger stationärer Behandlung auf Grund biochemischer, immunologischer und bioptischer Kontrollen, wobei die statistische Auswertung der biochemischen Leberwerte mit Hilfe eines Elektronenrechners durchgeführt wurde.
Histologisch fanden sich in der Leukerangruppe 4 gebesserte, 6 unveränderte und 3 verschlechterte Fälle, während in der Kontrollgruppe 5 Besserungen, 5 unveränderte Befunde und 1 Verschlechterung nachweisbar waren. Das Verhalten der Enzyme entsprach in etwa der Hälfte der Fälle nicht den histologischen Befundänderungen. — Insgesamt gesehen ließen die Kontrollergebnisse nach Therapie keine Überlegenheit der immunsuppressiven Behandlung erkennen. Unterschiede zwischen Versuchs- und Kontrollgruppe waren statistisch nicht zu sichern.
Es war auffällig, daß immunologische und histologische Befundänderungen bei 3 Fällen ein gegensinniges Verhalten zeigten. In etwas mehr als der Hälfte der Fälle waren trotz histologischer Anzeichen der Progredienz immunologische Phänomene vor und nach Therapie nicht nachzuweisen. In allen Fällen mit positiven Immunphänomenen ließ sich eine Rückläufigkeit der Immunreaktionen feststellen. Die Auswirkung der immunsuppressiven Therapie auf Granulo-und Thrombopoese und die damit zusammenhängende, noch ungelöste Frage der auch immunologisch ausreichenden Dosierung werden diskutiert.
Auf Grund der Untersuchungen wird gefolgert, daß sich die Indikationsstellung zur Anwendung von Immunsuppressiva bei chronischer Hepatitis streng nach den histologischen Kriterien der Progredienz und dem positiven Nachweis immunologischer Phänomene richten sollte.
Summary
In 24 patients with histologically proven chronic hepatitis the effect of an immunsuppressive treatment with Leukeran® was evaluated in a prospective study. 13 patients were given Leukeran, while 11 patients received Laevocholin and vitamines only. The therapeutic effects were judged on the basis of biochemical, immunological and biopsy controls after a 3 month course of hospital treatments. The results of the various liver function tests were statistically evaluated using a computer. On the basis of histologic criteria in the Leukeran group 4 patients were improved, 6 unchanged and 3 were worse. In the control group 5 patients were improved, 5 unchanged and 1 was worse. In about half of the cases there was no correlation between the serum enzym levels and the histological changes.
A significant difference between the two methods of treatment was not observed.
It is worth noting, that in 3 patients immunological and histological changes did not correlate at all. However, in all cases showing positive immunphaenomens those immunreactions were markedly reduced after the treatment with Leukeran. The observed side effects of immunsuppressive therapy on granulo- and thrombocytopoesis and their significance in regard to the problems of an immunological sufficient dosage are discussed.
