Zusammenfassung
Es wird über das Auftreten von Doppelalbuminämie in einer Sippe berichtet. Von 54 noch lebenden blutsverwandten Mitgliedern wurden 31 untersucht. Davon wiesen 17 (10 weibliche, 7 männliche) den Befund einer erbbedingten Doppelalbuminämie auf. Der Doppelalbuminämiebefund konnte mittels Acetatfolienelektrophorese-Untersuchung der Seren erhoben und als zum langsamen Typ gehörig gesichert werden. Die Papierelektrophorese-Untersuchung erbrachte keine eindeutige Trennung der beiden Albuminkomponenten, die in der CAF-Elektrophorese ein Flächenverhältnis von 6/5 (normalschnell/langsam wandernde Albuminkomponente) aufwiesen. In der Immunelektrophorese-Untersuchung zeigte sich auch bei Verwendung von Serumverdünnungen und im Ansatz gegen verschiedene Antihumanseren und monospezifische Antialbuminseren stets nur eine typische symmetrisch gebogene Form der Albuminpräcipitationslinie. Beide Albuminkomponenten konnten mittels kombinierter präparativer Kartonelektrophorese und Säulenchromatographie jeweils in elektrophoretisch, immunelektrophoretisch und sedimentationsanalytisch reiner Form gewonnen werden. Sie zeigten ein annähernd gleich großes S20W um 4,5 und eine immunologische Identitätsreaktion.
Die erhobenen Befunde werden den bisher in der Literatur beschriebenen Fällen gegenübergestellt. Der Begriff der Alloalbuminämie wird dem der Doppelalbuminämie übergeordnet, da er erlaubt, die heterowie homozygoten Fälle zusammenzufassen.
Summary
A family of 54 members distributed over four generations is described of which 17 kindred (10 females, 7 males) presented a hereditary bisalbuminemia. 31 members were studied. For diagnosis cellulose acetate electrophoresis was used and found to be very useful in detecting this albumin anomaly. By application of this method all affected subjects were found representative of the slow migrating type of the anomaly. By paper electrophoresis, no obvious separation of the two albumin components could be demonstrated, whereas cellulose acetate electrophoresis showed an area proportion of 6:5 (normal to slow-moving albumin components). Examination by immune electrophoresis constantly revealed one typical symmetrically-curved precipitation line of albumin, even when using serum dilutions, different antihumanserum-antisera and monospecific antialbumin-antisera. Both albumin fractions were prepared in an electrophoretically, immunoelectrophoretically and ultracentrifugally pure state by means of combined preparative cardboard electrophoresis and chromatography. They yielded similar sedimentation coefficients of about 4.5 (1% conc.) and immunologically identical reactions.
In this paper our results are compared with publications of cases previously reported. The term alloalbuminemia is preferred to that of bisalbuminemia, since it comprises both the hetero- and homocygote cases of the albumin anomaly.
