Zusammenfassung
Ein biochemischer Index zur Diagnose der Porphyria cutanea tarda (PCT) wird aufgestellt. Er resultiert aus der quantitativen Analyse der Porphyrine im Urin von 18 Patienten mit PCT. Uroporphyrin (U), Hepta(7)-, Hexa- und Pentacarboxyporphyrine sowie Koproporphyrin (K) wurden bestimmt. Eine PCT liegt vor, wenn folgende Konstellation in der relativen Verteilung der Porphyrine auftritt:U 46–77% und 7 19–30%. Der Anteil vonU+7 am Gesamtgemisch steigt durch 7 um durchschnittlich 28±11% (\(\bar x\)+2s,n=18) an. Der Korrelationskoeffizient zwischen den Verhältnissen 7:K und (U+7):K beträgtr=+0,988;r=+0,994 zwischen (K×100/U) und (K×100/U+7) (n=18). Der regelmäßige Anstieg des Heptacarboxyporphyrins ist spezifisch und signifikant für die PCT. Er wird bei anderen primären Porphyrien und sekundären Porphyrinurien nicht gefunden.
Summary
A biochemical index for the diagnosis of porphyria cutanea tarda (PCT) is established. It was computed from quantitative analysis of porphyrins in the urine of 18patients with PCT. Uroporphyrin (U), hepta(7)-, hexa- and pentacarboxylic porphyrins, as well as coproporphyrin (C), were assayed. PCT is indicated by the following constellation in the relative distribution pattern of the porphyrins:U 46 to 77% and 7 19–30%. The percentage ofU+7 in the total mixture increases due to 7 by an average of 28±11% (\(\bar x\)±2s,n=18). The correlation coefficient between the ratios 7:C and (U+7):C amounts tor=+0,988;r=+0,994 between (C×100/U) and (C×100/U+7) (n=18). The regular increase of 7 is specific and significant for PCT, and is not found in any other primary porphyrias or secondary porphyrinurias.
