Summary
Persistent adrenal insufficiency is considered a serious complication of corticosteroid therapy in man. Experimental findings in the rat and in patients will be discussed in which this side effect of steroids was prevented by therapeutic applications adapted to the circadian rhythmicity of ACTH.
The physiological peak of the ACTH and corticosteron secretion in the beginning of the activity phase in rats was depressed by giving dexamethasone 4 hours earlier during the rest period. The same steroid dose, however, had no effect on this endocrine rhythm when given 1 hour after the hormone secretion peak. In a long-term treatment (15 days), decreased hormone levels in blood and a significant adrenal atrophy was found only in the first group of rats, while the second group showed no alterations of the absolute hormone levels in blood, the rhythmicity and the adrenal weight, as compared to controls. In patients who had for many weeks been treated with 20 mg prednisone in 3 single doses throughout the day, the circadian rhythm of ACTH and cortisol secretion was abolished and the absolute blood levels decreased. In contrast, patients receiving the same dose of steroids, but in a single dose in the morning only, showed no modification of the same parameters in comparison with untreated volunteers.
The physiological role of circadian rhythms in the regulation of the hypothalamus-pituitary-adrenal system and the influence of exogenous steroids are discussed. For therapy in man, it is concluded that steroids should be administered only in a single dose in the early morning.
Zusammenfassung
Eine schwerwiegende Komplikation der Therapie mit Glucocorticoiden stellt die persistierende NNR-Insuffizienz dar. Es werden experimentelle Befunde bei Ratten und Patienten diskutiert, bei denen diese Nebenwirkung der Steroide durch eine der zirkadianen Rhythmik von ACTH angepaßte Therapie verhindert wurde.
Der physiologische Tagesgipfel der ACTH- und Corticosteron-Sekretion zu Beginn der Aktivitätsphase von Ratten wurde nur dann unterdrückt, wenn Dexamethason 4 Std vorher in der Ruhephase verabreicht wurde. Die gleiche Steroid-Dosis blieb dagegen ohne Einfluß auf die Tagesrhythmik, wenn die Injektion 1 Std nach dem Tagesgipfel erfolgte. Im Dauerversuch über 15 Tage fanden sich nur bei der ersten Versuchstiergruppe erniedrigte Hormonblutspiegel und eine signifikante Atrophie der NN, während bei der zweiten Versuchstiergruppe sowohl die absoluten Hormonblutspiegel und die Tagesrhythmik als auch das NN-Gewicht sich nicht signifikant von unbehandelten Kontrolltieren unterschieden.
Patienten unter einer Langzeittherapie mit Prednison in über den Tag verteilten Einzeldosen zeigten ebenfalls eine aufgehobene Rhythmik und gleichzeitige Erniedrigung der absoluten ACTH- und Cortisol-Blutspiegel. Dagegen fanden sich völlig normale Verhältnisse wie bei unbehandelten Probanden, wenn die gesamte Steroid-Dosis in den Morgenstunden eingenommen wurde.
Die Rolle der zirkadianen Rhythmik im Funktionskreis des Hypothalamus-HVL-NNR-Systems und ihre Beeinflussung durch exogene Steroide werden diskutiert. Für die Therapie des Menschen ergibt sich die Forderung, Glucocorticoide nur täglich einmal am frühen Morgen zu verabreichen.
