Summary
The effects of a single intravenous injection of 12.1 mg guanethidine base on both blood pressure and urinary catecholamine excretion were compared in 5 patients with essential hypertension. The intravenous administration of guanethidine decreased norepinephrine excretion, whereas epinephrine excretion in the urine remained unchanged. Diastolic blood pressure in supine and standing position was considerably lowered after the injection of the drug. Urinary creatinine excretion was not altered.
In 6 hypertensive patients, the influence of a 3-week's treatment with oral doses of guanethidine on the urinary excretion of norepinephrine and epinephrine was studied. At doses ranging from 20.2 to 80.8 mg/day, blood pressure in supine and standing position as well as urinary norepinephrine excretion were decreased. Urinary excretion of epinephrine and creatinine, however, remained unchanged.
These data as well as the time course of the effect on blood pressure and on norepinephrine excretion are compatible with the hypothesis that also in man the antihypertensive effect of guanethidine is due to a decrease of norepinephrine release from peripheral sympathetic nerves. Neither after a single intravenous injection nor after a 3-week's oral treatment with guanethidine was there an increase in urinary catecholamine excretion simulating phaeochromocytoma.
Zusammenfassung
Bei 5 Patienten mit essentieller Hypertonie wurde der Einfluß einer einmaligen Injektion von 12,1 mg Guanethidinbase auf Blutdruck und Katecholaminausscheidung im Harn verglichen. Nach der Injektion des Antihypertensivums kam es zu einem Abfall der Noradrenalinausscheidung bei unveränderter Adrenalinausscheidung. Der am liegenden und am stehenden Patienten gemessene systolische und diastolische Blutdruck fiel nach Injektion des Pharmakons deutlich ab. Die Kreatininausscheidung im Harn änderte sich nicht.
Bei 6 Hypertonikern wurde der Einfluß einer 3 Wochen dauernden Behandlung mit Guanethidin auf die Ausscheidung von Noradrenalin und Adrenalin im Urin geprüft. Die verabreichten Dosen zwischen 20,2 und 80,8 mg Guanethidinbase pro Tag senkten den am liegenden und am stehenden Patienten gemessenen systolischen und diastolischen Blutdruck. Unter Guanethidintherapie nahm die Noradrenalinausscheidung im Harn ab, während die Ausscheidung von Adrenalin und von Kreatinin sich nicht änderte.
Die geschilderten Befunde sowie der nach Guanethidin-injektion beobachtete Zeitverlauf von Blutdrucksenkung einerseits und Veränderung der Noradrenalinausscheidung andererseits sind vereinbar mit der Vorstellung, daß Guanethidin auch beim Menschen durch Verminderung der Noradrenalinfreisetzung aus peripheren sympathischen Nerven blutdrucksenkend wirkt.
Weder nach einmaliger intravenöser Injektion noch bei einer 3 Wochen dauernden Behandlung mit oralen Guanethidindosen wurde bei den untersuchten Hypertonikern eine vermehrte Katecholaminausscheidung im Harn festgestellt. Eine Änderung der Ausscheidung von Noradrenalin und Adrenalin, welche ein Phäochromocytom vortäuschen könnte, trat demnach bei Verabreichung von Guanethidin nicht ein.
