Summary
Evaluation of the histologic changes of the bone marrow and clinical data of twelve male and two female patients with hairy cell leukaemia before treatment. Mean age of the patients was 46 years, time from the onset of symptoms 5.5 months.
13 of the patients were anaemic, showing splenomegaly, 12 suffered from thrombopenia, 9 from granulocytopenia, and 8 from hepatomegaly. In all of the cases, the relative numbers of lymphocytes in the blood had been increased together with various amounts of characteristic hairy cells.
All of the cases had a typical histologic picture of lymphocytic bone marrow infiltration, mostly of the diffuse type. In 50% of the cases not only the well known rod-like intracellular inclusions could be seen, but also ring-shaped figures whose significance is discussed. Decrease of the granulopoiesis, disintergration of the marrow sinusoids, and osteoporosis are the most important additional signs. The progression of the disease is marked by increasing bone marrow infiltration, by splenohepatomegaly, anaemia, thrombopenia, and increasing numbers of typical lymphocytes in the blood. The bone marrow being considered to be the origin of the disease for good reasons, the histobiopsy of this organ ranks among the diagnostic and prognostic measures to be taken at first sight.
Zusammenfassung
Bei 14 unbehandelten Patienten (12 ∝, 2 ∝) mit Haarzell-Leukämie wurden nach Myelotomie am Beckenkamm und Einbettung in Methacrylat 3µ Dünnschnittserien hergestellt. Die morphometrisch-histologischen Ergebnisse und klinischen Befunde wurden ausgewertet. Das Durchschnittsalter betrug bei Myelotomie 46 Jahre, der durchschnittliche Verlauf 5,5 Monate (13 Fälle), 1 Fall 36 Monate.
Klinisch zeigten 92% (13/14) der Fälle eine Splenomegalie, 57% (8/14) der Fälle zusätzlich eine Hepatomegalie. Ausgeprägte periphere Lymphknotenvergrößerungen wurden in keinem Fall festgestellt. Blutchemisch zeigten 92% (13/14) der Fälle eine normochrome Anämie, 86% (12/14) der Fälle eine Thrombopenie unter 100000/mm3, 75% (9/14) der Fälle eine Granulozytopenie unter 8000 Leuko/mm3. Im Differentialblutbild betrug der Anteil von Lymphozyten und Haarzellen zwischen 40 – 94 rel.-%. Die Enzyme GOT, GPT, LDH zeigten nur in einem Fall eine leichte Erhöhung.
Histologisch zeigen alle Fälle eine teils diffuse, teils herdförmige Infiltration des Knochenmarks durch kleine mononucleäre Zellen mit rund-ovalem, teilweise stark gelapptem Zellkern mit Ausbildung eines feinen Retikulinfasernetzes. In 50% (7/14) der Fälle konnten stäbchen- und bisher nicht beschriebene ringförmige zytoplasmatische Einschlüsse in den Haarzellen lichtmikroskopisch nachgewiesen werden, die möglicherweise ein weiter fortgeschrittenes Krankheitsstadium anzeigen. Phagozytose von Erythrozyten oder Eisen wurde nicht beobachtet. Die typischen haarartigen Zytoplasmafortsätze sind histologisch selten zu beobachten. Von der normalen Myelopoese ist meist die Granulozytopoese frühzeitig besonders stark vermindert. Das spongiöse Knochenvolumen ist in allen Fällen gegenüber der Norm vermindert und nimmt mit Zunahme der Infiltration weiter ab. Das Sinussystem zeigt Wandauflösung mit Erythrozytenextravasaten. Folgende klinisch-hämatologische Befunde sind vom Ausmaß der Knochenmarksinfiltration abhängig: Hepatomegalie, Anämie, Thrombopenie und die Anteile von Lymphozyten und Haarzellen im peripheren Blut. Die Markinfiltration selbst ist ist abhängig vom Krankheitsfortschritt. Das Knochenmark ist wahrscheinlich der Ausgangsort der Störung; die histobioptische Knochenmarksuntersuchung ist die zur diagnostischen und prognostischen Beurteilung am besten geeignete Maßnahme.
