Paraproteinämie bei Hand-Schüller-Christianscher Erkrankung

Zusammenfassung

Es wird über einen Fall von autoptisch gesicherter Hand-Schüller-Christian'scher Erkrankung berichtet, bei dem klinisch auf Grund einer charakteristischen γ-Paraproteinämie bei deutlich ausprägter plasmazellulären Infiltration vor allem im Beckenkammtrepanat die Diagnose eines Plasmozytoms gestellt wurde. Auch eine intravital durchgeführte Leberpunktion ließ die Diagnose einer Hand-Schüller-Christianschen Erkrankung nicht stellen. Die Möglichkeit einer Paraproteinsynthese in den nachgewiesenen, morphologisch unauffälligen, plasmaretikulären Zellinfiltraten bzw. in den retikulo-histiozytären Zellproliferationen wird diskutiert.

Summary

A report of a proved case of Hand-Schüller-Christian disease in which an ante-morten diagnosis of plasmocytoma had been made because of a characteristic γ-para-proteinaemia with intense plasma cell infiltration especially striking in films from iliac trephining. Even ante-mortem liver puncture did not permit a diagnosis of Hand-Schüller-Christian disease to be made. The possibility of synthesis of paraproteins in definite but rather obscure reticular cells infiltrations or in the reticulo-histiocytic proliferations are discussed.
