Summary
Problems in the diagnosis and therapy of endometrial tumors in eight young women are discussed. Without knowledge of the patients' age the tumors from their histology would have to be classified as endometrial carcinomas. Thus, it is apparent that these tumors, although they may resemble one another, have quite different biological behaviours and consequently prognoses, depending on the age of the patient. Three patients had had a unilateral ovariectomy; three had received prolonged therapy with estrogen. In only three patients did the tumor at hysterectomy reveal its malignant nature by having invaded the myometrium. The remaining five patients never showed recurrence of the tumor; treatment in three of these merely consisted of curettage. In two patients the hysterectomy specimen proved the adenomatous overgrowth was limited to the endometrium. Various histological criteria, which must be checked for reliability in a large series of cases, seem to indicate that the tumors are reversible. Consequently, these subtile histological characteristics enable one to classify the major portion of juvenile “endometrial carcinomas” as reversible adenomatous hyperplasias, justifying conservative therapy for these young women. Because these tumors are rare, we suggest that they be collected at a central registry, in order to learn more about their biological behaviour and their prognosis.
