110. Die spontane Oesophagusruptur: Ein persönlicher Erlebnisbericht

Summary

This case was misdiagnosed for 11 days due to an atypical course. After vomiting the author had sudden excruciating pain in the lower left chest and epigastrium. A left hydropneumothorax followed 2 days later. Only 11 days after the event did a negative laparotomy and subsequent thoracotomy allow the correct diagnosis and treatment. The laceration of the esophagus was repaired and a transpleural drain was inserted. A small leak with mediastinal abscess, esophagopleural fistula and pneumonitis developed postoperatively and delayed recovery for 11 weeks.

Zusammenfassung

Das seltene Ereignis einer spontanen Oesophagusruptur erlitt der Verfasser aus voller Gesundheit und nahm einen atypischen Verlauf. Nach Erbrechen kam es zu heftigen Schmerzen im Oberbauch und der li. Thoraxhälfte. Erst 2 Tage später bildete sich ein linksseitiger Hypopneumothorax aus. Schließlich konnte am 11. Tag nach dem Ereignis durch eine explorative Laparotomie und anschließender linksseitiger Thorakotomie die Diagnose gesichert werden. Die Ruptur wurde übernäht und eine Pleuradrainage eingelegt. Der weitere Heilungsverlauf war durch eine pleurale Fistel und eine Pneumonie ü 11 Wochen verzögert.
