Summary
We found in a prospective study with standardized laboratory procedures: 1. Only amylase is of certain value as a single procedure (but 27 % false positive results in peritonitis); 2. The standard program can evaluate disturbed vital functions and can help to improve pre- and postoperative treatment; 3. Simultanously disturbed blood sugar, creatinine and acid-base balance mean a high risk; but these findings are signs of shock not of acute abdomen; 4. Laboratory procedures must not delay the necessary early operation.
Zusammenfassung
Laborbefunde können zur Klärung eines akuten Abdomens folgendes beitragen: 1. Als Einzeluntersuchung hat nur Amylase eine Bedeutung (jedoch 27 % falsch positive Befunde bei Peritonitis), 2. Ein Mindestspektrum erfaßt gestörte Vitalfunktionen und kann damit die prä- und postoperative Zusatztherapie verbessern; 3. Ist die DreierKombination BZ, Kreatinin und SBH pathologisch, bedeutet dies eine sehr ernste Prognose; diese Laborparameter sind nicht für das akute Abdomen spezifisch, sondern als Schockzeichen zu werten; 4. Keine Verzögerung der Frühoperation durch die Labordiagnostik!
