260. Welche Faktoren sollten die Indikationsstellung zur Leberresektion bei Metastasen colorectaler Carcinome beeinflussen?

Summary

We have analyzed the prognostic factors in 82 patients resected for colorectal cancer with liver metastases during the past 5 years. The operative mortality was 4.8%. The factor of highest significance on median survival time was the absence or presence of lymph node involvement of the primary tumor, both in synchronous and metachronous liver metastases. Besides this, the number of metastases (if less than four), the sex (men), and metachronous metastases are associated with improved survival times. Decrease of the CEA level to the normal range postoperatively is correlated with a prolonged, relapse-free interval, but not with better survival. The extent of the liver resection (if less than 50%) and the differential grade of the primary tumor had no influence on survival time.

Zusammenfassung

Die Analyse prognostisch wichtiger Faktoren bei 82 Patienten der letzten 5 Jahre, bei denen wegen metastasiertem colorectalem Carcinom eine Leberresektion durchgeführt wurde, ergab als relevantesten Faktor den Lymphknotenstatus des Primärtumors. Prognostisch signifikant günstiger waren weiterhin die Zahl der Metastasen (unter 4), metachrones Auftreten und das Geschlecht (Männer). Der Abfall des CEA-Spiegels postoperativ in den Normbereich korrelierte mit einem verlängerten rezidivfreien Intervall, jedoch nicht mit verbesserter Überlebenszeit. Das Ausmass des befallenen Parenchyms, Differenzierungsgrad des Primärtumors und Resektionsausmass waren nicht von signifikantem Einfluss. Die Operationsletalität betrug 4,8%.
