Summary
Five newborn infants, of whom three were premature, with hemorrhage into the abdominal cavity were seen during 1 year in the University Children's Hospital, Freiburg. Four infants had further sites of hemorrhage. In one patient severe defects of hemostasis could be demonstrated: Heparin tolerance and recalcification times prolonged, Quick test non-coaguable, prothrombin under 1%, Factor V 23%, Factor VII-complex 14%, Platelets 71 000/mm3. The hemorrhage could be controlled by substitutive therapy.
These observations stress a point of view as to the aetiology hitherto seldom taken into consideration: Bleeding into the abdominal cavity may occur as a manifestation of perinatal hemorrhagic diseases of the newborn. This is suggested particularly by the following criteria:

massive disturbance of hemostasis as demonstrated by blood coagulation studies;

2.
negative history of mechanical trauma to the abdomen during delivery;

3.
appearing of other bleeding sites;

4.
rapid hemostasis by specific therapeutic measures enhancing the blood clotting mechanism.
The therapeutic measures ensueing from these findings are briefly discussed.
Zusammenfassung
Von 5 Neugeborenen (3 unreife, 2 ausgetragene) mit Bauchhöhlenblutung, die im Verlauf eines Jahres in der Universitäts-Kinderklinik Freiburg i. Br. beobachtet wurden, hatten 4 weitere Blutungsmanifestationen. Bei einem Patienten wurde eine schwere Beeinträchtigung der Blutstillungsfunktion aufgedeckt (Heparintoleranzzeit und Recalcifizierungszeit verlängert, Quickwert ungerinnbar, Prothrombin unter 1%, Faktor V 23%, Faktor VII-Komplex 14%, Thrombocyten 71 000/mm3). Die Blutstillungsstörung ließ sich durch Substitutionstherapie beheben.
Auf Grund dieser Beobachtungen ergibt sich ein bisher wenig berücksichtigter Gesichtspunkt zur Ätiologie des Krankheitsbildes: Die Bauchhöhlenblutungen Neugeborener können als Manifestationsform der perinatalen Blutungsübel auftreten. Dies ist insbesondere dann anzunehmen, wenn

Gerinnungsphysiologische Untersuchungen eine massive Blutstillungsstörung aufdecken,
-
2.
Hinweise auf ein grobmechanisches Trauma unter oder nach der Geburt fehlen,
-
3.
zusätzliche Blutungen an anderer Stelle auftreten und
-
4.
eine spezifische blutstillungsfördernde Therapie rasch zum Stehen der Blutung führt.
Die sich aus diesen Erkenntnissen ergebenden therapeutischen Konsequenzen werden kurz besprochen.
