Intrathorakale Dislokation der Skapula bei oberer Rippenserienfraktur

Die intrathorakale Dislokation der Skapula ist eine Rarität, die noch seltener auftritt als die skapulothorakale Dissoziation. Für die korrekte Diagnosestellung sind die Rekonstruktion des Unfallhergangs, die Kenntnis der einwirkenden Kraft und die Anamnese Voraussetzung. Zu erwarten ist hierbei eine massive laterale oder dorsolaterale Gewalteinwirkung, die neben der Verletzung des Schultergürtels auch zu einer Impression der oberen und mittleren dorsolateralen Thoraxwand führt. Die Diagnostik sollte Röntgenaufnahmen der Schulter in 2 Ebenen, der Skapula tangential sowie des Thorax in ¶2 Ebenen beinhalten. Ergänzend sind eine Funktionsprüfung unter Durchleuchtung und ein thorakales Computertomogramm sinnvoll. Besondere Aufmerksamkeit sollten eine Fraktur des Schlüsselbeins oder eine Luxation seiner gelenkigen Aufhängung erregen, da nur bei diesen Verletzungen eine Dislokation der Skapula möglich ist. In Abhängigkeit von der Schwere des Thoraxtraumas ist die Wiederherstellung der dorsalen Thoraxwand zum frühestmöglichen Zeitpunkt anzustreben.

Intrathoracal dislocation of the scapula is rare, being even less common than scapulothoracic dislocation. Diagnosis requires reconstruction of the circumstances of the accident, knowledge of the force exerted and the anamnesis. When only a limited physical examination of the patient is possible the radiological diagnosis becomes proportionately more important. This should include plain films of the shoulder, and tangential files of the scapula and chest. Fluoroscopy of the shoulder function and computed tomography of the thorax should allow exclusion of intrathoracic dislocation of the scapula. With operative mobilization of the fixed scapula and sealing of the thoracic cavity good functional results can be obtained.
