Traumatic dislocation of the shoulder is caused by a high- or low-velocity trauma and leads to age-dependent soft tissue injuries. Concomitant bony and nervous injuries are more common than is generally expected. Adequate treatment, in addition to reduction, involves a detailed diagnostic and therapeutic strategy to prevent not only shoulder instability but also functional impairments. Rotator cuff tears in the elderly are still an unsolved problem, and the outcome is mostly poor even after surgery. With a view to lowering the recurrence rate, for acute dislocation of the shoulder in young patients an arthroscopic Bankart suture repair should be considered.
