Zusammenfassung
Ausgehend von den Befunden, daß eine intravenöse Gabe von Angiotensin II bei Normotonikern eine Antidiurese und Antinatriurese, bei Hypertonikern jedweder Genese dagegen in der Regel eine verstärkte renale Wasser- und Natriumausscheidung hervorruft, wird in Untersuchungen am Menschen zu klären versucht, inwieweit für diesen gegensätzlichen Effekt der Höhe des arteriellen Blutdrucks als solcher eine wesentliche Bedeutung zugesprochen werden kann. Zu diesem Zweck erhielten acht gesunde Freiwilligel-Noradrenalin als intravenöse Dauerinfusion in einer Dosis (8–18 µg/min), die den Blutdruck während der Dauer des Experimentes systolisch um 30–40, diastolisch um 15–20 mm Hg anhob. Unter diesen Bedingungen führte eine zusätzliche Applikation von nur 0,01–0,04 µg/kg/min Val5-Angiotensin II zu einer statistisch signifikanten Abnahme der renalen Wasser-, Natrium- und Chloridausscheidung. Nach Absetzen der Angiotensin-Infusion und unter Fortsetzung der Noradrenalingabe überschritt die Exkretionsrate der beiden Elektrolyte diejenige der einleitenden Kontrollperiode, obwohl das Harnzeitvolumen eine im Mittel nur geringe Tendenz zum Wiederanstieg zeigte. Die sich aus diesen Befunden für die Erklärung der eingangs genannten Wirkungsumkehr ergebenden Konsequenzen werden diskutiert.
Summary
It appears to be well established that angiotensin II reduces the renal water and sodium excretion in normotensive human beings, while in hypertensive patients it exerts an opposite effect: diuresis and natriuresis. It was the scope of the present study to investigate the effects of intravenously infused Val5-angiotensin II in healthy volunteers who were made hypertensive by the simultaneous administration ofl-noradrenaline. Even if the blood pressure was maintained at a level 30–40 mm Hg above normal by intravenous infusion of 8–18 µg/minl-noradrenaline, angiotensin still decreased in a dose range of 0,01–0,04 µg/kg/min statistically significant the renal excretion of water, sodium and chloride (8 experiments). After discontinuing the angiotensin but maintaining the noradrenaline infusion the sodium excretion rose above the pre-angiotensin period, while the water diuresis did not reach the initial values. The implication of these findings for the problem of altered angiotensin action in hypertensive patients is being discussed.
