Zusammenfassung
Nach Infusion einer glucosefreien Fettemulsion wurde der Abstrom des Fettes während und nach der Infusion bei Nierengesunden und Nierenkranken untersucht. Während der Infusion strömt das Fett mit etwa der gleichen Geschwindigkeit bei beiden Gruppen aus der Blutbahn ab. Nach beendeter Infusion verhielten sich die Abstromraten innerhalb der Nierenkranken uneinheitlich. Bei Nierenkranken mit normaler Lipidausgangskonzentration war sie normal, bei Patienten mit erhöhten Lipidwerten hingegen war sie nicht wesentlich von der Norm abweichend oder aber etwas verlangsamt.
Bei Gesunden und Nierenkranken nahm unter der Infusion die Kaliumkonzentration im Serum ab, desgleichen ging die Kaliumausscheidung mit dem Urin zurück. Die Natriumausscheidung war bei Gesunden ebenfalls vermindert, während sie bei Nierenkranken zunahm. Nierendurchblutung und Primärharnbildung nahmen in den ersten 20 min nach Infusionsbeginn um etwa 10–20% ab; danach stiegen sie auf den Ausgangswert bzw. darüber an.
Summary
In normal persons and in patients with renal diseases the disappearance of intraveneous admistered fat and his influence on serum electrolytes (Na; K) was investigated. The neutral fat disappeared from the bloodstream nearly with the same half time in both groops. During and after the infusion of neutral fat in normals persons and patients showed a decrease in plasma potassium concentration. The sodium content of plasma was not altered in normals in contrast to the fall of the sodium concentration in patients with renal diseases. During the fat infusion sodium excretion was decreased in normals whereas patients showed an increase in sodium output. In both groops there was a diminution of renal blood flow immediately after the start of the infusion.
