Zusammenfassung
An insgesamt 33 Patienten wurde der Einfluß der i.v. verabreichtenβ-Receptoren blockierenden Substanzen Propranolol, 1-(3-Methylphenoxy)-2-hydroxy-3-isopropylamino-propan (MHIP; Kö 592) und N-Isopropyl-p-nitrophenyläthanolamin (INPEA) auf Herzfrequenz und Herzminutenvolumen untersucht und mit derβ-adrenolytischen Wirkung verglichen. Als Maß für denβ-adrenolytischen Effekt wurde die Hemmung der positiv chronotropen Wirkung von 2µg Isoproterenol i.v. bestimmt.
Propanolol (5 mg), MHIP (5 mg) und (−)-INPEA (50–75 mg) antagonisierten die positiv chronotrope Isoproterenolwirkung und verminderten Herzfrequenz und Herzminutenvolumen.
Folgende Untersuchungsergebnisse sprechen gegen einen Zusammenhang vonβ-adrenolytischen Wirkungen und dem Einfluß auf Herzfrequenz und Herzminutenvolumen derβ-Receptorenblocker:

In einem Dosisbereich von 0,5–5 mg verlief die Dosis-Wirkungs-Kurve für dieβ-adrenolytische Wirkung von Propranolol und von MHIP wesentlich steiler als die Dosis-Wirkungs-Kurven für den Einfluß der beidenβ-Receptorenblocker auf Herzfrequenz und Herzminutenvolumen.

Dieβ-adrenolytische Wirkung von 50 mg (−)-INPEA war nicht mit dem Einfluß auf die Herzfrequenz bzw. auf das Herzminutenvolumen korreliert.

50 bis 75 mg (−)-INPEA hatte eine deutlicheβ-Receptoren blockierende Wirkung, während (+)-INPEA in der gleichen Dosierung nichtβ-adrenolytisch wirksam war. Beide optischen Isomere von INPEA senkten die Herzfrequenz und das Herzminutenvolumen jedoch gleich stark.
Summary
In 33 patients, a comparison was made of the actions of the beta adrenergic blocking agents propranolol, 1-(3-methylphenoxy)-2-hydroxy-3-isopropylamino-propane (MHIP; Kö 592), and N-isopropyl-p-nitrophenylethanol amine (INPEA). The effects on the following parameters were studied: heart rate, cardiac output, and the positive chronotropic action of 2µg isoproterenol. All drugs were given intravenously.
Propranolol (5 mg), MHIP (5 mg), and (−)-INPEA (50 to 75 mg) decreased heart rate, cardiac output, and the positive chronotropic action of isoproterenol.
From the following results it is concluded that the beta adrenergic blocking activity is not related to the decrease of heart rate and cardiac output in man:

In doses ranging from 0.5 to 5 mg the dose-response curve of the beta adrenergic blocking actions of propranolol and MHIP had a much steeper slope than the dose-response curves of the effects on heart rate and cardiac output.

There was no correlation of the beta adrenergic blocking action of 50 mg (−)-INPEA with effects on heart rate and on cardiac output.

50 to 75 mg (−)-INPEA antagonized the positive chronotropic effect of isoproterenol, whereas 50 to 75 mg (+)-INPEA had no beta receptor blocking activity. However, the effects of both optical isomers of INPEA on heart rate and on cardiac output did not differ.
