Zusammenfassung
Mit einem an 1 Tag durchführbaren Renin-Stimulations-Suppressionstest, der aus einer 2stündigen forcierten Natriurese mittels Fursemid und einer anschließenden 4stündigen intravenösen Natriumbelastung besteht, lassen sich die Störungen der Regulation der Reninsekretion bei Aldosteronmangel, primärem und sekundärem Aldosteronismus gut erfassen. Beim Aldosteronmangel (M. Addison) steigt die Plasmareninaktivität bei forcierter Natriurese höher an als bei Gesunden, fällt aber als Zeichen eines intakten Regulationsmechanismus während einer Natriumbelastung wie beim Gesunden unter den Ausgangswert ab. Beim primären Aldosteronismus bleibt die Plasmareninaktivität von Veränderungen der Natriumbilanz unbeeinflußt. Beim sekundären Aldosteronismus mit Ödemen (Lebercirrhose, Nephrose) führt Natriummangel zu einem Anstieg der Plasmareninaktivität, die aber im Gegensatz zum Verhalten des Gesunden bei Natriumbelastung nicht unter den Ausgangswert absinkt. Bei nichtödematösem sekundärem Aldosteronismus infolge einer Nierenarterienstenose waren mit der gewählten Versuchsanordnung keine Störungen der Regulation der Reninsekretion nachweisbar. Bei 2 Patienten mit einem nichtödematösen sekundären Aldosteronismus infolge einer Anorexia nervosa bzw. einem Bartter-Syndrom war bei hoher basaler Reninsekretion ein starker Anstieg des Plasmarenins während einer forcierten Natriurese zu beobachten, was dem Verhalten der gegenüber einem Natriummangel besonders empfindlichen Kranken mit Aldosteronmangel (M. Addison) entspricht.
Summary
The distrubances in the regulation of renin secretion with can be observed in patients with primary adrenal insufficiency, as well as during primary and secondary aldosteronism, can well be studied during a 2 hours' forced natriuresis caused by administration of Fursemid and followed by an intravenous sodium infusion lasting 4 hours. When Morbus Addison is present, the activity of renin caused by forced natriuresis increases further than it does in healthy subjects, descending however as in normal persons below the initial level during sodium over-loading, which suggests that the mechanism of regulation must be intact. In the case of primary aldosteronism, alterations of the sodium bilance have no effect in the plasma activity of renin. When secondary aldosteronism with edema (cirrhosis of the liver, nephrosis) is present, sodium deficiency leads to elevation of renin activity which, contrary to the observation made in healthy subjects, does not descend below the initial level during sodium loading. In the case of secondary aldosteronism without edema by renovascular hypertension our experiment did not show any disturbances in the regulation of the renin secretion. Two patients suffering from secondary aldosteronism, without edema, caused by anorexia nervosa or Bartter-syndrome, showed a marked elevation of plasma renin during forced natriuresis, accompanied by high basal renin secretion. This finding corresponds to the response of patients suffering from aldosterone deficiency, who are especially sensitive to sodium deficiency.
