Summary
Results of radioiodine therapy (131/125) of hyperthyroidism are reported. The individually calculated doses of 131-iodine were: 6000 rad in cases of small (not more than 60 g) diffuse goiters, 8000–10000 rad in large diffuse goiters, 10000–12000 rad in nodular goiter and 30000–40000 rad in autonomous adenoma.
In about 75% of the patients a single application of 131-iodine was sufficient to control the hyperfunction. The cumulative risk of myxedema ten years after therapy was: 31% in small diffuse goiters, 12% in large diffuse goiters and 6% in nodular goiter. All cases considered together, the cumulative myxedema risk was about 12%. This figure compares favourably with the results of surgical treatment and they document the necessity of individual calculation of the dose in order to get optimal results.
125-iodine was applied since October 1972 in cases of small diffuse goiters. The dose was standardized according to the gland weight, uptake and effective half value time (dose at the colloid-cell interface 12000–15000 rad). 5–12 months thereafter, there were about 60% of the patients euthyroid.
The genetic and somatic risk of radiation is considered quantitatively and we conclude that there is no real argument against radioiodine therapy of hyperthyroidism except in children and in youth.
Risks and efficiency of other treatments (surgical, antithyroid drugs) are compared with the radioiodine treatment and it is concluded that radioiodine is to be preferred in the major part of the hyperthyroid patients.

Es wird über die Behandlungsergebnisse der Hyperthyreosetherapie mit 131- und 125-Jod bei 2054 Patienten berichtet. Die Dosierung erfolgte individuell in rad. Dosierung bei 131-Jod: Struma diffusa unter 70 g: 6000 rad, Struma diffusa mit Gewicht über 70 g: 8000–10000 rad. Struma nodosa 10000–12000 rad. Autonomes Adenom: 30000–40000 rad.
Beseitigung der Hyperthyreose durch einmalige Applikation wurde erzielt: Bei 75% der Patienten mit Hyperthyreose mit Struma diffusa/nodosa und bei 85% der Patienten mit autonomem toxischen Adenom, bei 25% der Patienten mit Hyperthyreose mit Struma diffusa/nodosa und bei 17% der Patienten mit autonomem Adenom war mehrfache Therapie zur Erreichung der Euthyreose erforderlich. Faßt man alle Hyperthyreosen zusammen, so beträgt die Myxödemrate 10 Jahre nach Therapie mit 131-Jod etwa 12%.
Aufgeschlüsselte Myxödemraten nach 10 Jahren (weiterer jährlicher Zuwachs):
Struma diffusa unter 70 g: 31% (3,1%)
Struma diffusa über 70 g: 15% (1,2%)
Struma nodosa: 6% (0,7%).
In der von Becker (1971) zusammengestellten Statistik (über 5000 Fälle) liegt die kumulative Myxödemrate nach operativer Therapie bei etwa 25% nach 10 Jahren. Die Notwendigkeit der individuellen Dosierung wird durch den Vergleich unserer Ergebnisse mit Literaturergebnissen bestätigt: Bei ungezielter Dosierung besteht ein hohes Myoxödemrisiko, bei gezielter Dosierung ist das Mxyödemrisiko nicht größer als nach Operation. Das höhere Myxödemrisiko kleiner diffuser Strumen besteht bei jeder Therapieform, insbesondere auch bei chirurgischer Therapie. Aus theoretischen Erwägungen sollte die Therapie mit 125-Jod bei diesen Schilddrüsen die Myxödemrate verringern.
Die 125-Jod-Therapie wurde von uns bei Hyperthyreose mit Struma diffusa unter 60 g angewandt. Die Dosierung wurde auf der Grundlage der Berechnungen von Greiget al. so gewählt, daß am Kolloid-Zell-Interface etwa 12000–15000 rad deponiert wurden: 5–12 Monate danach war bei etwa 60% der erstmals behandelten Patienten die Hyperthyreose beseitigt. Die Beurteilung der Stellung der 125-Jod-Therapie der Hyperthyreose benötigt allerdings noch eine längere Beobachtungsdauer.
Aus der Diskussion des genetischen und somatischen Risikos der Strahlenbelastung bei Radiojodtherapie einer Hyperthyreose folgt, daß dieses Risiko zahlenmäßig kein reales Argument gegen die Radiojodtherapie ist, mit Ausnahme ihrer Anwendung bei Kindern und Jugendlichen und Schwangeren.
Effizienz und Risiken der alternativen Behandlungen der Hyperthyreose werden diskutiert.
