Abstract
1. O2-pressure and CO2-pressure, pH, standardbicarbonate and base excess of the coronarvenous and arterial blood were determined in 10 healthy male adults during rest, defined easy and heavy work and recovery.
2. In rest the coronarvenous pressure is 25,1±2,87 mm Hg. This values, which are directly determined by the platinic elektrode, are higher than the values, which were calculated by other authors under assistance of the O2-dissociation curve. It is possible, that the O2-dissociation curve of the coronary blood is displaced to the right.
3. During submaximal and maximal work no significant decrease of the coronarvenous O2-pressure is observed. It is concluded, that the coronarvenous O2-pressure has no importance for the regulation of the coronary circulation in healthy men during work.
4. After work a high significant increase of the coronarvenous O2-pressure was observed. The reason for this phenomenon is, that in the recovery the coronary circulation is higher than the really demand of the metabolism of the heart.
5. During rest the coronarvenous CO2-pressure is 48,4±5,17 mm Hg. During work the coronarvenous CO2-pressure and the arteriocoronarvenous difference of the CO2-pressure increase, because the production of CO2 by the heart is higher and the CO2-affinity of the blood smaller.
6. In spite of the high extraction of fixed acids (lactate, pyruvate, free fatty acids) the pH in the coronarvenous blood is higher than in the arterial blood because the CO2-production by the heart causes an increase of the CO2-pressure in the coronarvenous blood.
7. In rest, during and after work standardbicarbonate and base excess are higher in the coronarvenous blood than in the arterial blood and their changes are in quantitative agreement with the extraction of fixed acids.
Zusammenfassung
1. Bei zehn gesunden Männern im Alter von 20–30 Jahren wurden im coronarvenösen und im arteriellen Blut in Ruhe, während definierter steady state-Belastung und in der sich anschließenden Erholungsphase Sauerstoff- und Kohlensäuredruck, pH, Standardbicarbonat und base excess bestimmt.
2. Der normale coronarvenöse Ruhe-O2-Druck beträgt 25,1±2,87 mm Hg. Dieser auf direkten Messungen mit der Platinelektrode beruhende Wert ist gegenüber denjenigen Werten, die aus O2-Sättigungsmessungen anderer Autoren mit Hilfe der O2-Dissoziationskurve zu gewinnen sind, höher. Es ist möglich, daß die O2-Dissoziationskurve des Coronarvenenblutes eine Lageverschiebung nach rechts aufweist.
3. Während körperlicher submaximaler und maximaler Belastung konnte kein signifikanter Abfall des coronarvenösen O2-Druckes beobachtet werden. Daraus wird geschlossen, daß der Sauerstoffdruck im coronarvenösen Blut keine Regulationsgröße für die Coronardurchblutung bei körperlicher Arbeit darstellt.
4. Nach Belastung kommt es zu einem hochsignifikanten Anstieg des coronarvenösen O2-Druckes, der als Folge einer in der Erholungsphase über den tatsächlichen Stoffwechselbedarf des Herzens hinausgehenden Durchblutung aufgefaßt wird.
5. Der normale coronarvenöse CO2-Druck beträgt in Ruhe 48,4±5,17 mm Hg. Bei körperlicher Arbeit steigt der coronarvenöse CO2-Druck und die coronarvenösarterielle CO2-Druckdifferenz an, was als Folge des unter Arbeit gesteigerten Stoffwechsels des Herzens bei gleichzeitig verminderter chemischer CO2-Bindungsfähigkeit des Blutes anzusehen ist.
6. Der pH-Wert ist im Coronarsinusblut trotz der großen Extraktion fixer Säuren durch den Herzmuskel (Milchsäure, Brenztraubensäure, freie Fettsäuren), durch den gleichzeitig erfolgenden CO2-Druckanstieg in Ruhe, während Belastung und in der Erholungsphase niedriger als im arteriellen Blut. Der coronarvenöse Ruhe-pH-Wert beträgt 7,394±0,020.
7. Standardbicarbonat und base excess liegen in Ruhe, während Belastung und in der Erholungsphase in quantitativer Übereinstimmung mit der nachgewiesenen Extraktion fixer Säuren durch den Herzmuskel im coronarvenösen Blut jeweils höher als im arteriellen Blut.
