Lumbosacral dumb-bell neurilemmoma approached by microsurgical interlaminar foraminotomy

Summary.

The case history of a patient with a dumb-bell neurilemmoma arising from the L5 nerve root is described. The tumour extended into the outlet of the neural foramen at L5-S1 on the left and was also compressing the dural sac and the S1 nerve root. A limited laminotomy at L4-5 and L5-S1, with preservation of the neural arch, was followed by microsurgical medial foraminotomy at L5-S1 which allowed visualisation of the nerve root and enucleation of the tumour. The technique maintains stability of the posterior elements and spinal movement, and is recommended for the removal of this type of tumour.

Résumé.

Cette communication concerne une patiente âgée de 33 ans ayant subi une foraminotomie interne par microchirurgie pour ablation d’un neurinome de type «dumb-bell» (en forme d’haltère) placéà la racine nerveuse L5. La tumeur s’était développée jusqu’à la position de sortie du neuroforamen à L5-S1 sur la gauche et comprimait le sac dural ainsi que la racine nerveuse S1. Une laminotomie partielle aux niveaux L4-5 et L5-S1 avec préservation de l’arc neural postérieur suivie d’une foraminotomie interne par microchirurgie au niveau L5-S1 ont permis un accès total à la racine nerveuse affectée et donc à l’énucléation complète de la tumeur. Une foraminotomie par microchirurgie permettant de préserver la stabilité de l’elément postérieur et le mouvement de la colonne vertébrale est l’une des options conseillées pour l’ablation d’une tumeur intra-foraminale lombo-sacrée.
