CT scanning versus myelography in the diagnosis of lumbar stenosis

Summary

To assess the diagnostic accuracy of CT scanning in lumbar stenosis, twenty-nine patients in whom water-soluble myelography showed degenerative bony encroachment or suggested developmental narrowing of the spinal canal were scanned at all lumbar levels, and CT images were compared with myelographic findings. In developmental stenosis computerised tomography was found to be less accurate than water-soluble myelography, whereas in most cases of degenerative stenosis CT findings correlated well with myelographic patterns. An intermediate accuracy index was found in combined stenosis. In central degenerative encroachment of the spinal canal surgical treatment may be undertaken on the basis of CT findings. In the preoperative evaluation of the other types of lumbar stenosis CT scanning with 8-mm thick slices represents a useful adjunct to myelography, but cannot replace this investigation.

Résumé

Afin d' estimer la précision diagnostique de la tomodensitométrie en cas de sténose lombaire, 29 malades chez lesquels une myélographie aux produits de contraste hydrosolubles avait montré une protrusion osseuse d'origine arthrosique ou avait suggéré un rétrécissement congénital du canal rachidien, ont été étudiés des coupes étagées à tous les niveaux lombaires et les images ont été comparées aux résultats de la myélographie. Dans les cas de sténose congénitale la tomodensitométrie s'est avérée moins précise que la myélographie, tandis qu'on a trouvé une bonne corrélation entre les deux examens dans les cas de sténose dégénérative. Une fiabilité moyenne a été retrouvée dans les sténoses mixtes. En présence d'une protrusion ostéophytique, l'indication d'une intervention chirurgicale peut être posée sur la base des constatations tomodensitométriques. Lors du bilan pré-opératoire des sténoses lombaires d'autres types, la tomodensitométrie représente à l'heure actuelle un complément utile à la myélographie mais ne peut la remplacer.
