Zur Klassifizierung atypischer paraproteinämischer Hämoblastosen

Zusammenfassung

An Hand von 8 eigenen Beobachtungen (einem Fall von polyblastischer makroglobulinämischer Hämoblastose, 4 paraproteinämischen Retikulosen, einerHand-Schüller-Christianschen Erkrankung mit Paraproteinämie, einem makroglobulinämischen Plasmozytom und einer malignen Lymphoretikulose ohne Paraproteinämie) und unter Berücksichtigung des einschlägigen Schrifttumes wird die Problematik der atypischen paraproteinämischen Hämoblastosen aufgezeigt. Sie ergibt sich aus der Mannigfaltigkeit der von der “klassischen” Befundskonstellation des Plasmozytoms und der MakroglobulinämieWaldenström abweichenden Kombinationen des eiweißchemischen und morphologischen Bildes. Die Klassifizierung der jeweils vorliegenden Form der paraproteinämischen Hämoblastose sollte nur unter Berücksichtigung einer eingehenden morphologischen und eiweißanalytischen Untersuchung (Papier- und Immunelektrophorese, Ultrazentrifuge) erfolgen. Da bei den atypischen, paraproteinämischen Hämoblastosen eine befriedigende Zuordnung zum Krankheitsbild des Plasmozytoms oder der Makroglobulinämie Waldenström eben nicht möglich ist, empfiehlt sich die Formulierung einer deskriptiven Diagnose, die serologischen und morphologischen Befund in gleicher Weise umfaßt.

Summary

The problems of a typical paraproteinaemic haemoblastosis are discussed in the light of the relevant literature and of the eight cases personally observed (One case of polyblastic macroglobulinaemic haemoblastosis, four paraproteinaemic reticuloses, one example of Hand-Schüller-Christian disease accompanied by paraproteinaemia, one macroglobulinaemic plasmacytoma and one malignant lymphoreticulosis without paraproteinaemia.) The manifold features of the ‘classical’ syndrome and plasmacytoma and Waldenström's macroglobulinaemia vary in their protein chemistry and morphology. Classification of the known type of paraproteinaemia should only be done after full investigation-morphological and protein chemical (paper and immunoelectrophoresis, ultracentrifugation). As it is not possible to place the syndrome of plasmacytoma or of Waldenström's macroglobulinaemia in a satisfactory classification, it is advisable to do no more than give a descriptive diagnosis with the relevant serological and morphological data.
