Zusammenfassung
Chromosomale und zytochemische Untersuchungen wurden bei 3 Kranken mit Osteomyelosklerose durchgeführt. In 2 Fällen handelte es sich um eine “primäre” Marksklerose, in dem 3. Fall entstand die Marksklerose aus dem Bilde einer chronisch-myeloischen Leukämie. In allen 3 Fällen bestand eine Leber- und Milzvergrößerung, im peripheren Blutbild traten rote und weiße Vorstufen auf und histologisch, in einem Fall auch röntgenologisch, ließ sich eine Marksklerose bzw.-fibrose nachweisen.
Zytochemisch war die alkalische Leukozytenphosphatase erhöht. Es fanden sich vermehrt PAS-positive Erythroblasten und eine Erhöhung des Sideroblastenanteiles. Quantitative photometrische DNS-Messungen zeigten einen verminderten Anteil junger Blutzellen in DNS-Synthese.
In allen 3 Fällen fand sich ein Philadelphia-Chromosom, das nur in einem Fall klinisch durch das anfänglich bestehende Bild einer chronisch-myeloischen Leukämie erklärt werden konnte.
Die erhobenen Befunde gestatteten auch bei den 2 klinisch wie hämatologisch wahrscheinlich primären Fällen von Osteomyelosklerose keine Differenzierung gegenüber leukämischen Krankheitsbildern.
Summary
Chromosomal and cytochemical analyses were performed in three cases of osteomyelosclerosis. Two patients were classified as “primary myeloscleroses,” one exhibited a secondary form originating from chronic myeloid leukemia. All three cases showed enlargement of liver and spleen. Their blood specimens revealed immature stages of red and white cells. On the basis of histological and radiological studies (one case), they were diagnosed as myelosclerosis or myelofibrosis, respectively.
Cytochemical investigations demonstrated an increase of alkaline phosphatase in leucocytes, of PAS-positive erythroblasts, and of sideroblasts. DNA-values were determined by photometrical methods: in DNA-synthesis a reduced rate of early stages of blood cells was found.
Cytogenetical analyses of all three cases disclosed the characteristic Philadelphia-chromosome (Ph1). In one case only, this finding might be explained by the preceding chronic myeloid leukemia.
The results of these studies do not yield a differentiation between osteomyelosclerosis, primary or secondary, and leukemic processes.
