Resumen
Se comunica un caso de Histoplasmosis de evolución fatal en un Micólogo. La infección habia sido adquirida probablemente en el curso de trabajos de Laboratorio con cultivos del hongo. No fué factible excluír la posibilidad de una infección adquirida durante la recolección de muestras de tierra con Histoplasma capsulatum.
Mientras que la infeccion con Histoplasma capsulatum es un hecho frecuente como consecuencia de la exposición profesional, no han sido comunicados, a nuestro saber, casos de Histoplasmosis de evolución fatal entre Micólogos o personal de Laboratorio.
Se encontraron alteraciones tisulares producidas por el hongo, caracteristicas para esta micósis con multiples focos primarios pulmonares. Existia además una diseminación hematógena discreta en otros órganos.
Los granulomas histoplasmóticos se encontraron, en parte, cerca de paredes vasculares, con invasíde las mismas y trombosis de arterias pulmonares de mediano calibre con formación de infartos (Trombarteritis histoplasmótica).
En relación con las lesiones vasculares del pulmón, producidas por el hongo y aparentemente no descritas hasta la fecha, se discuten brevemente las complicaciones circulatorias conocidas en la Histoplasmosis.
La muerte súbita e inesperada se atribuye a una insuficiencia cardća aguda con shock provocados por la trombosis obliterante de las ramificaciones principales de la arteria pulmonar.
En la provocación de la muerte puede haber intervenido una esteatósico-necrosis aguda multíple ‘esencial”.
