10. Bedeutung szintigraphischer Untersuchungen für die Beurteilung von Folgezuständen nach Frakturen und Knochenoperationen

Summary

Scintigrams, obtained by administration of radioactive strontium, of fractures and bone operations will demonstrate a circumscribed increase of radioactive deposits, the intensity of which diminishes as the healing of the lesion progresses. Abnormally high and persistent deposition of radioactive strontium indicates the occurrence of complications in the healing process, which may be due to unfavourable positioning of the fragments, unstable fixation, formation of pseudarthroses, or infection. In atrophy caused by inactivity or in Sudeck's dystrophy scintigraphy will show a diffuse increase of radioactive deposits in the afflicted parts of the bone. Bone necroses are shown by corresponding disappearance of radioactivity in those areas around which bone transformation and thus deposition of radioactive strontium have increased. In accident surgery scintigraphy may supply valuable information when the situation is doubtful.

Zusammenfassung

Frakturen und Knochenoperationen führen auf Szintigrammen, angefertigt nach Radiostrontiumapplikationen zu einer umschriebenen Erhöhung der Radioaktivitätsablagerung, deren Intensität mit zunehmender Verheilung der Läsion sukzessive abklingt. Eine abnorm hohe, anhaltende Radiostrontiumalblagerung deutet auf das Auftreten einer Komplikation im Verheilungsverlauf, bedingt durch eine ungünstige Fragmentstellung, unstabile Fixation, Pseudarthrosenbildung oder Infektion. Auch bei einer Inaktivitätsatrophie oder Sudeckschen Dystrophie läßlt sich szintigraphisch eine diffuse Vermehrung der Radioaktivitätsablagerung in den befallenen Knochenpartien feststellen. Knochennekrosen imponieren bei entsprechender Größenausdehnung als Radioaktivitätsaussparungen, in deren Umgebung der Knochenumbau und damit die Radiostrontiumablagerung erhöht ist. In der Unfallchirurgie können szintigraphische Untersuchungen vor allem bei Vorliegen unklarer Situationen wertvolle Informationen liefern.
