195. Die Resektion von Pharynx - Larynx und Oesophagus beim Hypopharynx-Carcinom

Summary

Five patients with hypopharyngeal carcinoma infiltrating the esophagus, or with an additional tumor in the esophagus, were treated with total pharyngolaryngectomy, resection of the esophagus, and gastric or colon transposition. Tumors extending close or below the f. jugularis require an esophagectomy, and reconstruction is performed with pedicled stomach or colon. The high incidence of secondary synchronous or metachronous tumors in the esophagus must be taken into consideration for an effective oncologic procedure. When tumors have their inferior border 3 cm or more superior to the f. jugularis, the thoracic esophagus can be preserved. Reconstruction should be carried out with a free visceral graft from a jejunal loop.

Zusammenfassung

5 Patienten mit Hypopharynx-Carcinom und Infiltration oder Zweitcarcinom des Oesophagus erhielten eine totale Pharyngo-Laryngektomie mit Oesophagektomie und Magenhochzug bzw. Coloninterponat. Reicht die Tumorausbreitung bis in Höhe oder unterhalb der F. jugularis, ist eine Oesophagektomie indiziert. Die Häufigkeit von synchronen oder metachronen Zweittumoren im Oesophagus (14% aller diagnostizierten Doppeltumoren im oberen Aerodigestivtrakt) muss in die Indikationsstellung einer radikalen Tumorchirurgie mit einbezogen werden. Bei Tumorinfiltration bis etwa 3 cm oberhalb der F. jugularis kann die Rekonstruktion mit einem freien Jejunumtransplantat erfolgen.
