Summary
In 54 patients with excessive obesity a silicone balloon was implanted endoscopically into the stomach. The overweight amounted to 76% (median) by Broca's formula. The therapeutical plan included balloon-implantation, dietary treatment, psychotherapeutical guidance, kinetotherapy and balloon-extraction. According to their therapeutical compliance there were three different groups of patients: G1 — maintenance of therapy; G2 — discontinuation of therapy; G3 — no additional therapy at all after balloon implantation. Up to the sixth week a weight reduction of 9.5–20.5 kg could be demonstrated in all groups. Only in the first group a further weight reduction by a mean of 20 kg after 20 weeks was noted. Patients in group 2 and 3 — in some the balloon was still implanted — had a renewed weight increase sometimes surmounting the original weight.Complications: 3 gastric respectively duodenal ulcers, one subileus. The long-term success can only be obtained by a multi-component therapy plan.
Zusammenfassung
Bei 54 Patienten mit einer Adipositas permagna wurde ein auffülbarer Silikonballon endoskopisch in den Magen implantiert. Das relative Übergewicht nach Broca betrug 76%. Der Therapieplan umfaßte die Ballonimplantation, Diätberatung, psychotherapeutische Beratung, Bewegungstherapie and die Ballonextraktion. Nach der Therapiecompliance differenzierten sich 3 Gruppen (G): Gl — Patienten, die den Gesamttherapieplan einhielten. G2 — Patienten, die vor Ballonextraktion die weiteren Therapiemaßnahmen abbrachen. G3 — Patienten, die von Anfang an nach Ballonimplantation keine zusätzlichen Therapiemaßnahmen in Anspruch nahmen. Bis zur 6. Woche war in allen Gruppen eine Gewichtsreduktion von 9,5-20,5 kg nachweisbar. Nur in der Gl war eine weitere Gewichtsreduktion im Durchschnitt von 20 kg auch noch nach 20 Wochen zu verzeichnen. Patienten der G2 und G3 nahmen nach 6 Wochen zum Teil bei liegendem Magenballon an Gewicht zu bzw. überschritten ihr Ausgangsgewicht.Komplikationen: 3 Ulcera ventriculi bzw. -duodeni; ein Subileus. Die Applikation des Magenballons ist eine kurzfristig wirksame Therapiestütze, die im Sinne eines Füllkörpers im Magen die Funktion eines Appetitzüglers übernimmt. Ein langfristiger Erfolg ist nur durch Einhaltung eines vielseitigen Therapieplanes zu erzielen.
