Abstract

1.
The half-life of passively transfused IgM in newborn infants was determined by assying the clearance in infants undergoing exchange transfusion. It was found to be 10 days. Birth weight and IgM level after exchange transfusion did not influence catabolism.

The examination of 2 infants with coli-septicaemia and 6 with staphylococcus septicaemia showed that the rate of synthesis of IgM can be accelerated two- to sixfold.

High values of IgM after the 4.–5. day of life do not allow the diagnosis of a connatal infection, as this could have been acquired post-natally.

Durch Bestimmung des IgM-Schwundes im Blut von 3 Neugeborenen, bei welchen postnatal eine Austauschtransfusion durchgeführt werden mußte, ließ sich eine Halbwertszeit für das passiv zugeführte IgM von etwa 10 Tagen berechnen. Die Halbwertszeit war unabhängig vom Geburtsgewicht und vom IgM-Wert unmittelbar nach Austauschtransfusion.

Es konnte an Hand von Untersuchungen bei 2 Neugeborenen mit Colisepsis und 6 Neugeborenen mit Staphylokokkensepsis gezeigt werden, daß unter den Bedingungen eines perinatalen Infektes die IgM-Syntheserate zwei- bis sechsmal höher sein kann als bei gesunden Neugeborenen.

Erhöhte IgM-Werte im Blut von Neugeborenen berechtigen nach dem 4.–5. Lebenstag nicht mehr dazu, einen konnatalen Infekt anzunehmen, es kann sich in diesen Fällen auch um postnatal erworbene Infektionen handeln.
