Abstract
Quantitative determinations of the serum alpha1-antitrypsin (α1-ATR) were made with Partigen-and M-Partigen® plates in 113 normal newborns and infants, 42 newborns with hyperbilirubinemia of different origin, and 25 newborns with sepsis (caused by: streptococcus, staphylococcus, pneumococcus, pseudomonas aeruginosa, listeria monocytogenes, E. coli, and Klebsiella). Within the first days after birth no change of α1-ATR were detected in the normal newborns. New-borns with sepsis in most cases showed a temporary increase of α1-ATR when compared with normal newborns. In some cases we could find pathological values at the first appearence of clinical symptoms.
The quantitative α1-ATR determination presents a further diagnostic facility to prove or exclude sepsis during infancy. This method, however, has its limits, since in some cases of sepsis, caused by the bacteria mentioned above, false negative results can be expected (values within the 2-σ range for normal infants) in 20% of the cases. Furthermore, some newborns with hyperbilirubinemia who do not have sepsis will show pathologically elevated values.
Zusammenfassung
Bei 113 gesunden, 42 Neugeborenen mit Ikterus verschiedener Genese und 25 Neugeborenen und jungen Säuglingen mit Sepsis (hervorgerufen durch: Streptokokken, Staphylokokken, Pneumokokken, Pyocyaneus, Listerien, Coli und Klebsiellen) wurden mit Partigen-bzw. M-Partigen®-Platten quantitative Bestimmungen des Serum-Alpha1-Antitrypsins durchgeführt. Innerhalb der ersten Lebenstage waren bei gesunden Neugeborenen keine Änderungen des Alpha1-Antitrypsins festzustellen. Das arithmetische Mittel lag innerhalb der ersten 7 Lebenstage bei 293 mg% (obere 2σ-Grenze: 430 mg%), es sank bis zum 44. Lebenstag auf 250 mg% (obere 2σ-Grenze:370 mg%). Bei Neugeborenen mit Rh-und ABO-Inkompatibilität überschritten die Alpha1-Antitrypsinspiegel die obere 2σ-Grenze gesunder Kinder in 14 von 22 Fällen, bei Neugeborenen mit Hyperbilirubinämie ohne Inkompatibilität in 6 von 20 Fällen. Neugeborene und junge Säuglinge mit Sepsis zeigten in den meisten Fällen zumindest vorübergehend eine Erhöhung des Alpha1-Antitrypsins im Vergleich mit Gesunden. In Einzelfällen konnten bereits beim Auftreten klinischer Symptome pathologische Werte nachgewiesen werden.
Die quantitative Alpha1-Antitrypsinbestimmung stellt eine weitere diagnostische Möglichkeit zur Sicherung bzw. zum Ausschluß einer Sepsis im frühen Kindesalter dar. Eine Einschränkung erfährt die Methode dadurch, daß in Sepsisfällen, die durch die erwähnten Keime verursacht sind, 20% falsch-negative Resultate (Werte innerhalb der 2σ-Grenzen für gesunde Kinder) zu erwarten sind. Außerdem werden auch bei Kindern mit nicht durch Sepsis bedingten Hyperbilirubinämien pathologisch erhöhte Werte gefunden.
