Zusammenfassung
An 29 Neugeborenen und Säuglingen wurden i.v. Fettinfusionen (Intralipid®, Lipofundin®) mit einer Infusionsgeschwindigkeit von 0,5 bis 1,5 g Fett/kg KG/Std verabreicht und Gesamtfette, Gesamtcholesterin, Elektrolyte und Brenztraubensäure an einigen, an allen Kindern die pH-Werte, das \(P_{CO_2 } \), das Basendefizit (Standardbicarbonat, aktuelles Bicarbonat, Gesamt-CO2) und die Milchsäurekonzentrationen bestimmt.
Es scheint bedeutsam, daß bei den untersuchten Kindern wesentlich häufiger als bei Erwachsenen (Steinbereithner u. Vagacs) nach großen i.v. verabreichten Fettgaben eine pasagere metabolische Acidose auftrat. Dabei kam es gleichzeitig zu einer Abnahme der Milchsäurekonzentration im Blut.
Biochemisch mögliche Erklärungen werden aufgezeigt und diskutiert.
Summary
Intralipid® and Lipofundin® were given as intravenous drop infufions to 29 infants. The pH, \(P_{CO_2 } \), alkali deficit (standard bicarbonate, actual bicarbonate, total CO2) and lactic acid levels were determined in all patients, while the total lipids, total cholesterol, electrolytes and pyruvic acid only in some.
The fact that a metabolic acidosis of short duration following the i.v. lipid infusions had a more frequent occurance in these infants than in adults (Steinbereithner and Vagacs) seems noteworthy. Asimultaneous drop of the lactic acid level was also observed.
Possible biochemical explanations are mentioned and discussed.
