Hypothenar-Hammer-Syndrom als seltene Ursache eines Raynaud-Syndroms

Zusammenfassung

Vorgestellt wird ein 25jähriger Zimmermann mit schweren Raynaud-Attacken an den drei lateralen Fingern der rechten Hand unter Kälte und Vibrationen, bevorzugt auftretend unter einer Kombinationsbelastung aus Schwingungen und Kälte, wie z.B. bei der Arbeit an der Kreissäge im Winter. Die von uns durchgeführten Spezialuntersuchungen bestätigten den Verdacht auf ein Hypothenar-Hammer-Syndrom, nachdem zuvor eine Behandlung mit Prostavasininfusionen, Acetylsalicylsäure und Pentoxifyllin im Heimatkrankenhaus unter der Verdachtsdiagnose einer Thrombangiitis obliterans erfolglos verlaufen war. Angiographisch waren bereits fortgeschrittene Gefäßveränderungen die Digitalarterien und der A. ulnaris nachweisbar. Die sorgfältige differentialdiagnostische Abgrenzung gegenüber der Thrombangiitis obliterans und anderen, klinisch ähnlich imponierenden Krankheitsbildern mit lokalem Raynaud-Syndrom ist von entscheidender prognostischer Bedeutung. Angesichts der potentiell invalidisierenden Gefäßveränderungen muß auf eine Nichtverwendung der Hand als Hammer sowie eine Vermeidung von vibrierendem, handgeführtem Arbeitsgerät geachtet werden.

Summary

A 25-year-old carpenter with severe localised Raynaud syndrome of the lateral three fingers of his right hand is described. The patient reported digital ischaemia caused by cold or vibration, and especially when both were present simultaneously. He was initially diagnosed with thrombangitis obliterans, but failed to respond to intravenous prostaglandin therapy, pentoxifyllin or aspirin. Diagnostic procedures performed in our hospital, especially angiography, confirmed our suspicion of hypothenar hammer syndrome. Radiological examination revealed severe anatomical and functional changes of the distal ulnar artery and, digital arteries of the affected digits. The differential diagnosis of Raynaud syndrome should include the hypothenar hammer syndrome as a possible cause. Early diagnosis and prevention of hand trauma is the only possible waý to stop progression of this disease, which leads to invalidity.
