Bei der Behandlung kindlicher Frakturen hat sich in den letzten Jahren zunehmend die sogenannte halbkonservative, minimalinvasive Therapie etabliert. Besonders mit der elastisch-stabilen Markraumschienung (Prevot-Nagelung) steht ein schonendes Verfahren zur Verfügung, das langwierige Ruhigstellungen überflüssig macht. Bei korrekter Indikation und Anwendung erfüllt sie das Ziel einer primär definitiven Therapie und einer möglichst schnellen Mobilisierung der Kinder. Die wesentliche Indikation liegt in der Behandlung von Schaftfrakturen. Aber in einigen Sonderfällen, wie der Radiushalsfraktur oder der subkapitalen Humerusfraktur, kann sie die Kirschner-Draht-Spickung ersetzten. Ein besonderer Problemkreis sind die Frakturen im Ellbogenbereich, insbesondere die suprakondyläre Humerusfraktur. Grob dislozierte, instabile Frakturen müssen nicht selten offen reponiert und mit Kirschner-Drähten fixiert werden.

The main indication is shaft fracture. In fractures of the radial neck or of the proximal humerus it can be a substitute for Kirschner wire fixation. Particular problems are encountered with fractures of the elbow, especially supracondylar fractures. Grossly displaced and unstable fractures often require open reduction and Kirschner wire fixation.
