Zusammenfassung
Besteht bei einem kreuzbandinsuffizienten Kniegelenk eine symptomatische Instabilität, so ist in aller Regel die operative Stabilisierung indiziert. Das heutige gängigste Verfahren ist die Kreuzbandersatzplastik aus körpereigenem Material, z. B. Patellarsehne oder Semitendinosussehne. Diese Transplantate verlieren während der Umbauphase einen großen Teil ihrer Festigkeit und sind dadurch einer erhöhten Ruptur- bzw. Elongationsgefahr ausgesetzt. Hier besteht die Möglichkeit, durch ein synthetisches Band die „biomechanische Stabilitätslücke“ zu überbrücken. Während die alleinige Naht eines gerissenen Kreuzbands wenig Erfolg verspricht, gibt es für die Kreuzbandreinsertion unter dem Schutz des Kunstbands positive klinische und experimentelle Daten. Allgemeine Vorbehalte gegen künstliche Bänder sind Materialermüdung, Unverträglichkeit, Förderung der Arthrose und Verhinderung eines adäquaten Umbaus des biologischen Transplantats. Diese Problematik wird in der Literatur kontrovers diskutiert. Während der Einsatz von synthetischen Bändern als Kreuzbandersatz weitgehend abzulehnen ist, haben diese als temporärer Schutz einer Kreuzbandnaht oder eines Kreuzbandtransplantats, unter der Voraussetzung, daß das richtige Band gewählt wird, durchaus ihre Berechtigung.
Abstract
ACL-deficient knees with symptomatic instability generally require operative stabilisation. The most widely accepted surgical procedure is currently replacement of the ruptured ACL with autogenous material, preferably the patellar tendon (BTB) or the semitendinosus tendon. These transplants, however, are subject to necrosis during remodelling, causing a major reduction of their strength with a consequent increased risk of rupture or elongation. Augmentation with a synthetic ligament can be helpful to bridge this “biomechanical gap”. While suture of ¶a ruptured ACL alone does not yield good long-term results, there are positive clinical and experimental data showing that reinsertion of the ACL with protection by an alloplastic ligament will restore knee stability. General reservations against synthetic ligaments are based on material fatigue, lacking biocompatibility, progression of arthrosis, and interference with transplant remodelling. These problems are discussed from all angles in the literature. While replacement of the ACL with an artificial ligament alone (prosthesis function) is not acceptable, temporary augmentation of a suture or of an autogenous transplant is justified, provided always that the correct synthetic ligament is chosen.
