Résumé
Les auteurs décrivent un appareil stéréotaxique qu'ils ont utilisé pour porter une seule ou une série de lésions en correspondance des nucléus du thalamus et de l'hypothalamus, ou pour interrompre le faisceau spino-thalamique en correspondance du colliculus supérieur.
Avec cette technique ils ont traité 42 malades psychiques atteignant des bons résultats avec une mortalité du 3%.
Cette technique s'est démontrée particulièrement utile pour les syndromes douloureuses du visage et du thorax.
Enfin ils ont traité trois patients malades de grand et petit mal, un desquels a eu une amélioration remarquable.
Zusammenfassung
Die Verfasser beschreiben ein Stereoencephalotom, mit dem es möglich ist, eine Läsion, oder eine Reihe von Läsionen, von gradierbarer Intensität, an die Kerne des Thalamus und in den Hypothalamus zu bringen.
Nachdem der Apparat auf dem Kopfe des Kranken befestigt worden ist, wird eine Radiographie gemacht, wobei die in Frage kommenden Stellen festgehalten werden, wo der Apparat in derselben Weise und mit derselben Genauigkeit im Augenblick der Operation zu befestigen ist. Wenn die Pinealdrüse nicht sichtbar ist, ist ein Pneumoencephalus erforderlich. Die Verfasser haben diese Technik für die Behandlung einiger Formen von Geisteskrankheiten, bei unerträglich schmerzhaften Syndromen und bei gewissen Arten von krampfhaften Störungen angewandt.
Die bei 42 Kranken ausgeführte Thalamotomie und Hypothalamotomie für die Behandlung einiger Formen von Geisteskrankheiten, wobei kleine bilaterale Läsionen in den dorsomedialen Kernen des Thalamus oder in dem subthalamischen oder lateral-hypothalamischen Gebiete entstanden, ergab eine Sterblichkeit von 3%. Fünf Kranke wurden wieder arbeitsfähig; zwölf kehrten ohne völlige Arbeitsfähigkeit heim, acht blieben, wenngleich leichter umgänglich, im Krankenhaus; zehn blieben unverändert und acht hatten Rückfälle.
Es sind Studien über das Verstandsvermögen vor und nach der Operation gemacht worden und die E. E. G. sind mittels Elektroden registriert worden, die mit dem Apparat eingeführt wurden.
Mit demselben Apparat und mit derselben Technik ist die Gegend des oberen Colliculus behandelt worden, um die spinothalamische Leitungsbahn (Mesencephalotomie und Mesencephalothalamotomie) zu unterbrechen.
Die Verfasser glauben, daß der Eingriff besonders nützlich in Fällen von Facialschmerzen sei, bei denen mit anderen Eingriffen kein Erfolg erzielt worden war und bei Schmerzen des Armes und der Brust.
Läsionen mit dieser Technik wurden im dorsomedialen Kern des Thalamus bei drei Kranken produziert, die an Anfällen von grand et petit mal litten. In einem Fall war die Besserung offensichtlich und bestand aus einer geringeren Anzahl von Anfällen und einer verminderten Intensität der assozierten Motorphänomene.
Summary
The AA describe a stereotaxic apparatus, by means of which it is possible to bring one lesion or a series of lesions of modifiable intensity into the nucleus of the thalamus and into the hypothalamus.
The reference points are determined by a roentgenography made with the apparatus fixed on the head in the same manner and with the same exactness with which it will be fixed at the time of the operation. If the pineal body is not visualized, a pneumoencephalogram is necessary.
The AA have applied this technic to the treatment of some forms of mental diseases, of unbearable pain and of certain types of convulsive disorders.
Thalamotomy and hypothalamotomy for the treatment of some mental diseases, producing small bilateral lesions in the dorsomedial nucleus of the thalamus or in the subthalamic or lateral hypothalamic region, have been done by 42 patients, resulting in a mortality of 3%: five patients returned home with full working capacity; twelve returned home with diminished working capacity; eight remained in the hospital, but they were more easily manageable; ten were unchanged and eight have relapsed.
Studies have been made about the intellectual capacity before and after the operation and E. E. G. have been registered by means of electrodes introduced with the apparatus.
With the same apparatus and the same technic, lesions have been placed into the region of the superior colliculus for interrupting the spinothalamic tract (mesencephalotomy and mesencephalothalamotomy).
The AA believe that the operation is particularly useful in cases of facial pain, unrelieved by other operations, and by pain in the arm and chest.
With this technic, lesions were produced in the dorsomedial nucleus of the thalamus by three patients suffering from attacks of grand et petit mal. In one case the amelioration was evident, consisting in a diminished number of attacks, and in a decreased intensity of the associated motor phenomena.

Gli autori descrivono un apparecchio stereotassico di cui si sono serviti per portare una unica o una serie di lesioni in corrispondenza dei nuclei del talamo e dell'ipotalamo o per interrompere il fascio spinotalamico in corrispondenza del colliculus superior.
Con questa tecnica hanno trattato 42 malate psichici ottenendo buoni risultati e con una mortalità del 3%.
Particolarmente adatta si è dimostrata la tecnica per le sindromi dolorose della faccia e del torace.
Infine sono stati trattati 3 malati portatori di forme di grand et petit mal e uno di questi ne ha avuto un notevole vantaggio.
Resumen
Los autores describen un aparato estereotáxico (estereoencefalotomo) con el que pueden producir una serie de lesiones, de mayor o menor extensión, en los núcleos del tálamo e hipotálamo.
Los puntos de referencia se determinan con una radiografiá hecha con el aparato colocado en la cabeza, de tal modo que corresponda exactamente a la posición que ha de tener durante la intervencion.
Los autores han empleado esta técnica para el tratamiento de ciertas enfermedades mentales, en síndromes dolorosos y en determinadas formas de epilepsia.
La talamotomía e hipotalamotomía, produciendo lesiones peqeñas bilaterales en el núcleo dorsomedial del tálamo, en la region subtalámica o en las porciones laterales del hipotálamo, ha tenido una mortalidad de un 3% en una série de 42 enfermo. Cinco enfermos pudieron reanudar su trabajo, doce regresaron a su domicilio con una capacidad limitada para el trabajo, ocho permanecieron recluidos aunque algo mejorados, diez no se modificaron y ocho tuvieron recaídas.
Han hecho estudios sobre la capacidad intelectual antes y después de la intervención. Asimismo se han tomado registros eléctricos con electrodos introducidos en profundidad valiéndose del aparato en cuestión.
Con este mismo aparato y siguiendo la misma técnica han actuado sobre la región del colículo superior para interrumpir la via espino-talámica (mesencefalotomía y mesencefalotalamotomía).
Los autores creen que esta intervención tiene una indicacion especial en algias faciales que no han mejorado por otra intervención y en algias braquiales y de tórax.
Se empleó la misma técnica de actuar sobre el nucleo dorsomedial del tálamo en tres enfermos afectos de gran y petit mal. En uno de ellos hubo una mejoría evidente con disminución del numero de crisis y menor intensidad de su componente motor.
