Zusammenfassung
Wachsende Ratten wurden der Wirkung eines orthopädischen Gerätes unterworfen; dieses Gerät verursachte eine Unterkiefer-Propulsion durch eine im Oberkiefer fixierte Vorbißführung. Durch Erweiterung der seitlichen Teile des Gerätes um die Zunge und tief in Richtung des Mundbogenswurde die Beweglichkeit des Unterkiefers eingeschränkt. Die tägliche 12 Studen dauernde Behandlung mit dem LSU- oder demHerren-Typ- “Aktivator” verursachte einen minderen Zuwachs des Musculus pterygoideus lateralis, eine Reizung der Wachstumsgeschwindigkeit im Kondylenknorpel, eine entsprechende öffnung desStutzmann-Winkels, ein nachfolgendes Wachstumssupplement der Länge der Mandibula, vorausgesetzt, daß die Höhe der seitlichen Bißsperre einen gewissen Wert (1,6 mm in diesen Experimenten) nicht erreichte. Die tägliche Behandlung mit einem “Aktivator”, welcher eine 0,8 mm hohe seitliche Bißsperre hat, bewirkte einen minderen Zuwachs des Musculus pterygoideus lateralis, eine Reizung der Wachstumsgeschwindigkeit im Kondylenknorpel, eine Öffnung des Winkels zwischen der Hauptrichtung der Knochentrabekel im Kondylus und dem Mandibularplanum, ein Supplement des Wachstums der Mandibula, unter der Bedingung, daß die Dauer des Aktivatortragens nicht 22,5 Stunden, sondern nur 12 oder 18 Stunden beträgt. Bei Durchführung der Propulsion durch einen im Unterkiefer fixierten “Aktivator” wird nach 4 Wochen die Wachstumsgeschwindigkeit des Kondylenknorpels stärker gereizt, als wenn die Propulsion durch einen im Oberkiefer fixierten “Aktivator” erzielt worden ist. Die Öffnung desStutzmann-Winkels ist im Gegenteil größer, wenn das Gerät im Oberkiefer getragen wurde. Die Verlängerung des Unterkiefers ist nach Behandlung mit dem “Aktivator” beider Arten ungefähr gleich. Am 48. Tag des Experiments ist der Unterkiefer bei jungen Ratten länger, die der Wirkung des “Aktivators” tagsüber unterworfen wurden. Es muß betont werden, daß die Ratte keine dem Menschen ähnliche Schlafperiode, sondern einen vielphasischen Schlaf hat; die meisten Schlafphasen erfolgen bei Tage und nicht nachts. Wenn solche experimentellen Ergebnisse auf das Kind übertragen werden, wird ein “Aktivator” wirksamer sein, der vom Patienten in der Nacht getragen wird. Auch nach der Resektion der Musculi pterygoidei laterales wirkt der “Aktivator” auf Wachstumsgeschwindigkeit, Wachstumsrichtung und Wachstumsquantität des Unterkiefers ein. Die Ergebnisse einer klinischen Forschung haben gezeigt, daß die Unterkieferwachstumsgeschwindigkeit während der Behandlungszeit mit einem LSU- “Aktivator” beschleunigt war. Die Messung des staturalen Wachstums vor und nach Eingliederung des Aktivators wies keine systematische Zunahme des Zuwachses auf, so daß spontane Variationen der Knorpelwachstumsgeschwindigkeit für das Supplement der Unterkieferlänge nicht verantwortlich gemacht werden können.
Summary
Twenty day old male rats were treated for 4 weeks with aHerren-type or LSU- “activator” (R. Shaye) which was fixed to the maxilla. The appliance causes a forward mandibular positioning of 2.0 mm.The mandibular motility is restricted through the lateral portions of the appliance which are extended deeply toward the floor of the mouth. The animals were divided into several groups corresponding to variations in appliance height (0,4 mm, 0,8 mm, 1,6 mm) and differing amounts of daily appliance wear (12, 18, 22,5 hours). In a separate group, the external pterygoid muscles were resected before the activator was inserted. With a vertical increase of the molar bite extending from 0 (controls) to 1,6 mm, the greatest stimulation of the condylar cartilage growth rate and of the lengthening of the mandible was obtained for a 0,8 mm deviation. A 1,6 mm vertical deviation of the molar bite did stimulate in a less extend the growth rate of the condylar cartilage. The direction of growth of the condyle became more posterior with the increase of the vertical deviation produced by the “activator” up to 0,8 mm, but became rather more anterior for a 1,6 mm deviation. The number of serial sarcomeres in the lateral pterygoid muscle is inversely proportional to the magnitude of the vertical increase of the molar bite for the 0,4 mm and 0,8 mm deviation; but, with an “activator” inducing a 1,6 mm vertical deviation, the length of the lateral pterygoid muscle did not differ significantly from the controls. A lesser increase in the addition of sarcomeres during growth corresponds to an anatomically shorter lateral pterygoid muscle, which leads to a more forward positioning of the mandible. The greatest stimulation of the growth rate of the condylar cartilage in the mandible was observed when rats wore the activator for a 12 hours daily duration; it is still significant for a 18 hours daily treatment, but does not differ significantly from controls when the appliance was imposed for 22,5 hours. Longer the duration of the daily treatment, and greater the lengthening of the mandible up to 18 hour-daily treatment with the “activator”; the 22,5 hours wearing of the appliance did not stimulate the growth of the mandible. The direction of growth of the condyle exhibited parallel variations; it was significantly increased when the “activator” was worn for 12 or 18 hours, but the “Stutzmann's angle” was not modified for a 22.5 hours daily treatment. The length of the lateral pterygoid muscle, as compared with the controls, decreased in the rats treated for 12 and 22.5 hours. The stimulating effect of the activator on the growth rate of the condylar cartilage and on the lengthening of the mandible was detected even in rats whose lateral pterygoid muscle had been resected demonstrating that these muscles onlypartly mediate the action of the “activator”. TheLSU-type “activator's” action seems to imply a two-step effect. Indeed, during the time of wearing the appliance, the more forward positioning of the mandible is the cause of the reduced growth of the lateral pterygoid muscle; simultaneously a new sensory engram is formed for the new positioning of the lower jaw. During the time the activator isnot worn, the mandible is functioning in the more forward position in such a way that the menisco-temporocondylar fraenum will be much more stimulated than in controls. The increased iterative activity of the fraenum will produce an earlier beginning of the hypertrophy of the condylar chondroblasts and, consequently, an increased growth rate of the condylar cartilage. In other words, the lateral pterygoid muscle does mediate the action of the “activator”, but the stimulating effect on the condylar growth appears to be produced almost exclusively during the time the appliance isnot worn, whereas the effect of the postural hyperpropulsor, of theAndresen-Häupl, “activator”, of theFränkel appliance, and of class II elastics on the condylar cartilage growth is produced mostlyduring the wearing of the appliance. In a parallel investigation 34 human subjects treated with a similarly designed “activator” exhibited a highly significant increase in the mandibular growth rate when compared to a pre-treatment reference period.

Des rats mâles, âgés de 20 jours, ont été soumis pendant 4 semaines à l'action d'un “activateur” de typeHerren ou LSU, fixé sur le maxillaire. Le dispositif provoquait un postionnement de la mandibule décalé vers l'avant de 2 mm.La mobilité mandibulaire était limitée en raison des extensions latérales du dispositif en profondeur, qui atteignaient le fond de la cavité buccale. Les animaux ont été séparés en différents groupes correspondant à des appareils de hauteurs différentes et à diverses durées de traitment quotidien. Dans l'un des groupes expérimentaux, les muscles ptérygoïdiens latéraux ont été réséqués avant le traitement orthopédique. L'accélération de la vitesse de croissance du cartilage condylien est la plus élevée pour un “activateur” produisant un décalage vertical de 0,8 mm au niveau des molaires. Le décalage vertical de 1,6 mm a eu un moindre effet stimulant sur la croissance du cartilage condylien. La direction de croissance du condyle est plus postérieure après un décalage vertical de 0,4 et 0,8 mm, mais elle est plus antérieure après une surélévation de 1,6 mm. Le nombre de sarcomères dans le muscle ptérygoïdien latéral est inversement proportionnel à l'hauteur de la déviation verticale jusqu'à 0,8 mm; mais après une déviation de, 1,6 mm, la longueur du muscle ptérygoïdien latéral n'est plus différente de celle des témoins. Un moindré accroissement du nombre de sarcomères correspond à un muscle ptérygoïdien latéral plus court, ce qui conduit à un positionnement plus avancé de la mandibule. L'accélération de la vitesse de croissance du cartilage condylien la plus grande s'observe quand les rats ont porté l'“activateur” 12 heures; ell est encore significative après un traitement quotidien de 18 heures, mais n'est plus décelable lorsque l'appareil a été porté 22,5 heures. La longueur de la mandibule augmente à raison du nombre d'heures de traitement quotidien avec l'activateur, mais un dispositif porté 22,5 heures n'a plus d'effet. L'angle entre les travées osseuses néoformées sous le cartilage condylien et le plan mandibulaire s'ouvre après un traitement quotidien par l'activateur de 12 heures et 18 heures, mais n'est pas modifié par un traitement de 22,5 heures. La longueur des muscles ptérygoïdiens latéraux est toujours moindre après un traitement par l'activateur. L'effet stimulant de l'activateur sur la vitesse de croissance du cartilage condylien et sur l'allongement de la mandibule se retrouve même chez les rats dont les muscles ptérygoïdiens latéraux ont été réséqués; ce qui démontre que ces muscles ne sont qu'un intermédiaire parmi d'autres de l'action de l'“activateur”. Le mode d'action de l'“activateur” de typeLSU semble impliquer l'existence d'un effet-en deux temps. Pendant le port de l'appareil, le positionnement plus antérieur de la mandibule est à l'origine d'un moindre accroissement du muscle ptérygoïdien latéral; simultanément, un nouvel engramme sensoriel se constitue pour le nouveau positionnement de la mandibule. Pendant les heures où l'“activateur” n'est pas porté, la mandibule fonctionne dans une position plus avancée, de sorte que le frein ménisco-temporo-condylien est plus stimulé que celui des témoins. L'accroissement de l'activité itérative du frein produit une hypertrophie précoce des chondroblastes, et par conséquent, une accélération de la vitesse de croissance du cartilage condylien. En d'autres termes, le muscle ptérygoïdien latéral est un médiateur de l'action de l'“activateur”, mais l'effet stimulant sur la croissance condylienne semble se produire uniquement pendant les heures où l'appareiln'est pas en place, alors que les effets de l'hyperpropulseur postural, de l'activateur deAndresen-Häupl, de l'appareil deFraenkel, ou des élastiques classe II sur la croissance du cartilage condylien se produisentpendant le port du dispositif. Dans une recherche parallèle, 34 enfants traités avec des appareils similaires ont présenté une accélération significative de la vitesse de croissance mandibulaire par rapport à une période de référence antérieure au traitement.
