    Notizen: Abstract We conducted a retrospective study to evaluate the diagnosability of breast tumors after breast reductions as this is a frequent surgical procedure. The data should shed light on the hypothesis that routine screening methods concerning the diagnosis of breast tumors prove more difficult after breast operations. All women who had undergone breast reduction at our department between January 1989 and December 1994 were examined. During this period we counted 166 patients; the majority of them (n = 144) had undergone a bilateral breast reduction and the rest of them (n = 22) a unilateral breast reduction for various reasons. After the operation, all patients were checked in standardized intervals. Those who developed any kind of breast mass (n = 6) were recorded and examined by ultrasound and mammography, and occasionally by an additional fine-needle biopsy. In case any doubt about the dignity had remained, an excisional biopsy was carried out. In none of our patients was it possible to get a precise diagnosis of an ill-defined mass with ultrasound. With mammography, some of the existing masses, which were really scars, mimicked different kinds of tumors, and once a carcinoma was initially interpreted as scar tissue with oil cysts. The diagnosis of breast masses after breast reductions with routinely used screening methods has proved to be more difficult as breast reductions lead to architectural alterations of the remaining breast parenchyma. Such alterations can and should be documented shortly after the operation so that later occurring tumors are distinguished more easily. Therefore, a basic mammography 3 months after each breast reduction has to be claimed in order to facilitate further breast tumor diagnosis.

    Notizen: Abstract A reduction mammoplasty must produce a reduction in volume, a natural lasting shape, and minimal residual scarring. Many attempts to achieve this goal have been developed in recent years. The author described, in 1970, a vertical technique achieving an important reduction and a good shape but with the appearance of the end of the vertical scar below the brassiere line. In 1977, the author [3] modified the technique by the adjunction of a small horizontal scar to eliminate the inconvenience of the visible part of the vertical scar. In this article the author describes his technique which appears to be suitable for most types of breast deformities.

    Beschreibung / Inhaltsverzeichnis: Zusammenfassung 197 Mammareduktionsplastiken nach Strömbeck bei 100 Patientinnen wurden ausgewertet. Die adipöse Makromastie und die pendulierende sackförmige Brust waren die häufigsten Indikationen. Im Durchschnitt wurden 740 g Gewebe pro Scite entfernt, bei 18,2 % lag die Gewichtsreduktion pro Seite bei über 1000 g. Ein Ansteigen der Wundkomplikationsrate mit dem Ausmaß der Reduktion konnte auch in unseren Untersuchungen festgestellt werden. Das kosmetische Ergebnis war mit 80 gut bis sehr gut. Die Partnerbeziehung wurde durch die Reduktionsplastik in der Regel nicht verändert. Die mammografische Beurteilung bezüglich der Carcinomfrüherkennung war nach der Reduktionsplastik etwas erschwert, in 80% fand sich eine gute Beurteilbarkeit. Die körperlichen Beschwerden als wesentliche Ursache für die OP-Indikation konnten in allen Fällen beseitigt werden.
    Notizen: Summary Report about 197 breast reductions on 100 patients which were operated by the method of Strömbeck. Indication was mostly macromasty and excessive ptosis. In the mean 740 g were removed on each side, in 18.2 % there was removed more than 1000 g on one side. The more tissue was removed, the higher were local complications. The cosmetic result was good to excellent in 80%. The behaviour to the husband was in most cases not changed after the operation. Mammographic studies showed a good interpretation in 80%. After the operation all patients lost their preoperative complaints.

Der Brustwarzenhof ist deutlich empfindlicher als die Peripherie, der Brustnippel ist sensibel ganz schlecht versorgt. Postoperativ nimmt die Sensibilität bis 2 Jahre lang zu, ohne das Ausgangsniveau zu erreichen. Die Sensibilitätsminderung ist von der Operationsmethode (Strömberg/Pitanguy) unabhängig. Die Sensibilitätsminderung verhält sich proportional der reduzierten Gewebsmenge. Wichtig ist die Erhaltung der Stillfähigkeit (sympathische Versorgung durch den anterioren Ast des 4. Ramus cutaneus lateralis).
    Notizen: Summary All non-operated breasts have an irregular pattern of sensibility. The areola is distinctly more sensitive than its periphery; the nipple has a very low grade of sensitivity. The postoperative sensitivity increases for about 2 years without reaching the former level. The loss of sensitivity is unrelated to the method of operation (Strömberg, Pitanguy). The loss of sensitivity is proportional to the reduction of tissue. Most important is to keep the ability for breast-feeding (sympathetic innervation by the arterior branch of the 4th ramus cutaneus lateralis).

    Notizen: Summary Medial confluence of the breasts, or symmastia, is probably not a rare entity, despite the paucity of the literature on the subject. It causes aesthetic disfigurement and problems with apparel and, in the case presented, was associated with chronic furunculosis or multiple pilonidal sinuses. Both problems were satisfactorily dealt with by means of a modified bipedicled vertical dermal flap technique with excision of the central part of the chest wall, where the skin lesions were present. The result was cosmetically acceptable.
