Abstract A prospective clinical trial was performed to determine the accuracy of computer-assisted pedicle screw insertion in 30 consecutive low back operations. Thirty transpedicular titanium screws were implanted with computer assistance. The accuracy of screw placement was studied postoperatively using a sophisticated computed tomography protocol. A total of 174 pedicle screws were implanted, 139 (79.9%) of them with the navigation system. Lateral perforation of the pedicular cortex occurred in 4.3% of all cases; in the 35 screws inserted by the conventional technique the perforation rate was 14.3%. The difference in the perforation rate is statistically significant (χ2 test, P = 0.03). One screw inserted by the conventional technique caused L4 root paresis and had to be changed. The accuracy of pedicle screw placement with computer navigation proved to be superior to the accuracy obtained when conventional techniques were used. The author’s 4-year experience of clinical use of the navigation system and future developments are discussed.

Zusammenfassung Die klinische Anwendbarkeit eines Computersystems für die bildinteraktive Navigation bei der lumbalen Pedikelschraubeninsertion wurde in einer prospektiven Studie an 30 konsekutiven Patienten untersucht. Es wurden insgesamt 174 Pedikelschrauben implantiert. Bei 139 Schrauben (79,9%) konnte das Navigationssytem eingesetzt werden. Die Verifizierung der Schraubenlage erfolgte postoperativ durch einen unabhängigen Radiologen nach einem speziell entwickelten CT-Protokoll anhand von transaxialen Schnitten der Wirbel sowie Serienschnitten durch jeden instrumentierten Pedikel. Von 139 Schrauben lagen 133 (95,7%) perfekt innerhalb des Pedikels. Bei ¶6 Schrauben (4,3%) wurde eine laterale Perforation der Pedikelkortikalis bis maximal 4 mm registriert. Von 35 Schrauben, die ohne Computerassistenz eingebracht worden waren, perforierten 5 (14,3%) die Pedikalwand bis maximal 6 mm. Eine 5 mm medial fehlpositionierte Schraube verursachte Wurzelsymptome und mußte gewechselt werden. Die Differenz der Perforationsraten ist statistisch signifikant (χ 2 -Test, p = 0,03). Mit Hilfe der Computernavigation konnte eine signifikante Verbesserung der Genauigkeit und damit der Sicherheit der Pedikelschraubenimplantation im Lendenwirbelsäulenbereich erzielt werden. Praktische Erfahrungen und zukünftige Entwicklungen werden diskutiert.
