Summary After it had been possible in an earlier publication to demonstrate the usefulness of Inulin-131I for nephrography by means of the distribution pattern and renal time-activity curves, the present investigations deal with the useability of the preparation for the clearance determination. It was shown that adjustment of a blood concentration level is possible by external measurement of the activity above the heart. This guaranties for the first time a constant plasma level which is required for Inulin clearance. Furthermore, the classical clearance procedure seems to be replaceable by a radiomedical method. Calculation of the values follows from the Inulin-131I administration and plasma concentration at a constant blood level and filled-in Inulin volume. In further animal experiments and clinical investigations the value of this new method is to be established by parallel determinations of Inulin- and Inulin-131I clearance.

Zusammenfassung Nachdem in einer früheren Mitteilung die Brauchbarkeit von Inulin-131J für die Nephrographie anhand des Verteilungsmusters und der renalen Zeit-Aktivitätskurven demonstriert werden konnte, beschäftigen sich die hier vorgelegten Untersuchungen mit der Verwendungsmöglichkeit des Präparats für die Clearance-Bestimmung. Es zeigte sich, daß die Einstellung eines Blutspiegelplateaus durch externe Aktivitätsmessung über dem Herzen möglich ist. Damit ist erstmals die Einhaltung der für die Inulin-Clearance geforderten Konstanz des Plasmaspiegels gewährleistet. Darüber hinaus scheint das klassische Clearance-Verfahren durch eine nuklearmedizinische Methode ersetzbar zu sein. Die Errechnung der Werte erfolgt dabei aus der Inulin-131J-Eingabe und der Plasmakonzentration bei Blutspiegelkonstanz und aufgefülltem Inulinraum. In weiteren tierexperimentellen und klinischen Untersuchungen soll der Wert dieser neuen Methode durch Parallelbestimmungen der Inulin- und Inulin-131J-Clearance ermittelt werden.
