Summary Time and chance of functional regeneration after intra- and extratemporal operations of the facial nerve are discussed on the basis of 155 operations. Blood supply of the nerve has an essential importance. Consequently impeded regeneration has to be reckoned with mainly in cholesteatoma induced palsy — if damage of the bony base is present, — as well as in certain pyramid fractures involving circulatory changes. Although the spontaneous regenerative ability of the facial nerve is known this possibility has to be applied deliberately and not to be overtaxed.
